Responsibilities:
Responsible for the coordination, planning, development, implementation and evaluation of strategies, objectives, and actions based on the Healthy Living Program 2.0 grant. Develops strategic community/county partnerships that support the grant program initiatives. Ensures that strategic plans leading to better community health outcomes are achieved.
Qualifications:
Education
Bachelor's Degree in community health administration, or other health related fields.
Experience
3-5 years community health and/or grant program experience preferred.
