Job description
Responsible for ensuring potential members are interviewed to determine various grants and/or program eligibility. Ensures member accounts are managed that include assisting members with account summarization. Provides direct routine patient care and mentoring of clinical support staff.
DUTIES AND RESPONSIBILITIES:
1. Performs screenings on impending members for potential eligibility in various programs available through the Center; predetermination and assistance with application completion for special funded programs; accurately places clients on appropriate programs based on funding source requirements.
2. Performs chart preparation to facilitate patients being seen for tele-health visits.
3. Assists in the coordination of patient care and nursing activities; to include proper documentation of medical history, and vitalization. Ensures that all quality metrics are completed and documented in compliance with the organizations regulatory and accreditation agencies. Participates and prepares for the Centers internal/external audits. Screens and responds to member phone calls, directing to provider as needed. Ability to perform nursing duties in all areas.
4. Provides direct routine patient assessment and treatment, to include clinical/nursing procedures, as required within the scope of practice. Notifies provider as required for non-routine care or other unusual medical circumstance. Schedules urgent visits as appropriate. Processes medication refills per organization policies and procedures. Provides and promotes health education within area of specialty. Performs recall of patients to ensure compliance of laboratory and pharmacy orders.
5. Complies with departmental policies and funding source requirements; maintains current level knowledge of updates in funding source regulations.
6. Ensures accurate activations of plans and programs and records special notations in member account software; assists member with registration.
7. Collects and enters member information into the appropriate electronic practice management system to initiate member entrance into the Primary Care Medical Home. Documentation is to be concise, thorough, and accurate.
8. Maintains inventory of medical supplies, immunization and operating supplies. Ensures that all equipment is calibrated, charged, and clean. Responsible for waive testing in accordance to the manufacturer’s instructions.
9. Provides excellent internal/external customer service via telephone, fax or face-to-face contact to assist members with their health care needs.
10. Provides member education on eligible coverage for health care services as approved by the funding source; educates members on all other programs offered by the Center.
11. Performs other related duties as assigned.
JOB QUALIFICATIONS:
Graduate from an accredited school of vocational nursing
Current unrestricted Texas LVN/LPN license
Certified in Basic Life Support (BLS), American Heart Association
Bilingual in English/Spanish required
College degree in Healthcare Administration preferred
Experience with MS Office, Excel, EHR preferred
Scheduled hours and/or work locations are subject to change
Ability to travel overnight is required
Ability to work unsupervised in a professional manner at all times
Ability to plan, organize and perform well under stress
REQUIRED PHYSICAL AND MENTAL ACTIVITIES AND REQUIREMENTS:
Finger Dexterity: Using fingers to make small movements such as typing or picking up small objects. Normal fine and gross motor control of fingers and hands.
Talking: Frequently conveying detailed or important instructions or ideas accurately, clearly, or quickly.
Hearing: Able to hear average or normal conversations and receive ordinary information; hear whispered voice at five (5) feet.
Repetitive Motions: Frequently and regularly using the wrists, hands, and fingers.
Visual: Visual acuity necessary for the proper evaluation of a patient and to prepare or inspect documents or other materials. Ability to accurately view computer monitors
Physical: Sitting 20-30% of time and standing, walking 70-80% of time. Lift up to 25 lbs; push and pull up to 45 lbs. Able to bend at the waist, twist and turn along axial plane.

- Associates or better in Nursing
Licenses & Certifications
Required- Basic Life Support (BLS)
