Supportive Services Case Worker
Job description
The Alliance for Community Empowerment (formerly ABCD Company) is looking for caring, motivated Supportive Services Case Worker to join our team and support our community.
Supportive Services Case Worker is responsible for working with all Alliance for Community Empowerment, Inc. client/families to identify barriers and gaps in services and track outcomes to determine how customers are doing as a result of the services provider. Interview and enroll clients/families in our various programs. Must assist the family in its own efforts to improve conditions, receive services they need and are eligible for, and promote self-sufficiency through action plans. Work with client/family to develop personal action plan that identify individual/family goals. Work with clients to develop plans to help them achieve economic independence and self-sufficiency. Referral and follow-up activity with community resources in response to health and social service needs of clients/families. Follow up with clients at least every 90 days. Engage in advocacy work on behalf of individual/family. Write and Maintain extensive Case Notes on status and progress of each client. Able to learn and use the CAPTAINS systems case management & other software required by funding sources proficiently. Participate with an interdisciplinary team in regular case conference meetings to mobilize and coordinate services within and outside the agency on behalf of assigned clients/families. Be aware of Social Services and Health agencies and develop a working relationship with staff persons for a smoother transition when making referrals. Establish and coordinate a community resource data bank for families. Prepare required reports. Enter data into computer. Performs other tasks as assigned.
Education/Experience/Skills:
- Bachelor’s degree in Human Services or related field plus at least one year experience in Human Services OR Associate degree in Human Services or related field plus at least three years experience in Human Services. OR a High School graduate with five years experience in Human Services.
- Knowledge of Social Service/Health Agencies in the Greater Bridgeport area.
- Bi-lingual oral and written communication skills are helpful but not required.
- Strong verbal and interpersonal skills a must.
- Computer proficiency is required.
