Job Summary: Provides counseling services including identifying, screening and assessing clients to determine the need for substance use treatment. Provides referrals to community-based organizations and coordinates care as needed. Responsible for facilitating therapeutic group and individual sessions. Responsible for completing and maintaining detailed OASAS-compliant case records
Duties and Responsibilities:
· Conduct intake interviews, biopsychosocials, mental health screenings
· Formulate treatment plans and treatment plan reviews, act as participant’s case
manager to coordinate care within Crossroads and will external providers
· Perform individual counseling duties which include holding weekly individual
sessions with clients, providing referrals to community-based resources (healthcare, social services, and administrative e.g. help obtaining insurance), and developing client schedules
· Initiate and maintain client records in accordance with policies and procedures
Ensure accurate and timely data entry of all client information into the agency’s database; utilize electronic health record system
· Facilitate assigned treatment groups
· Perform drug testing as needed
· Prepare monthly updates for court or other supervising agencies
· Participate in treatment team case reviews.
· Consult with other professionals in support of client’s treatment
· Complete discharge documentation as needed
Qualifications:
· LMSW or CASAC is required (CASAC is also preferred for social workers)
· Minimum of two years’ clinical/case management experience in working with
clients that have substance use disorders is required
· Experience working with the criminal justice population is preferred.
· Computer literacy (word processing, e-mail, internet research, data entry) is
required; ability to use an electronic health record system required;
Bilingual (English/Spanish) preferred.
Job Type: Full-time
Pay: $54,000.00 - $60,000.00 per year
