SUMMARY:

Work as part of a team providing case management to a caseload of clients in the Supported Apartment Program (SAP). Work closely with Health Homes while engaging clients to improve their medical and mental health to ensure independent living within the community. Complete all required agency documentation including creating and maintaining client charts, and developing care plans. Responsible for the management of apartment maintenance and repair needs through the landlord.

RESPONSIBILITIES:

  • Escort clients to apartment viewings.
  • Maintain spreadsheets including client and rent information as well as the lease drive.
  • Co-facilitate quarterly case conferences with Health Homes.
  • Co-facilitate monthly community meetings.
  • Plan and participate in monthly socialization events.
  • Maintain a caseload and provide case management services for clients.
  • Assist clients with activities of daily living to promote long term housing stability.
  • Monitor housing stability, access to medical/mental health care and entitlements to a caseload of clients.
  • Monitor completion of safety and cleanliness inspections of apartments.
  • Payment of invoices due for services.
  • Maintain records on client income collectables.
  • Handle SAP monthly utility allowances for public assistance clients.
  • Handle SAP monthly reports including HRA PACT Turn Around Documents (TAD).
  • Purchase and maintenance of supplies.
  • Collect monthly program fees, rent collection and track all in weekly spreadsheet.
  • Handle rent submission in the absence of the Program Director.
  • Work collaboratively with the Operations team and Sr. Program Director to insure repairs and maintenance are done in a timely manner.
  • Maintain, and follow up with, Social Determinants of Health (SDOH) Surveys.

A Bachelor's degree is required and three years of housing, case management experience working with the homeless population or 5 years experience working in case management with a psychiatrically challenged population. Ability to establish workload priorities and balance diverse projects needed. Must have exceptional communication skills both orally and in writing for individuals with complex and sensitive issues. Physical ability to perform the essential job functions needed. In addition, the Sr. Housing Coordinator shall perform any duties as required to meet the needs of the program and the clients we serve.
