Job description
For Dialysis: Responsible for gathering information about the Chronic Dialysis patient's social, psychological, cultural, environmental, and financial situation. Counsels patients and families regarding the adjustment to their illness, treatment, and lifestyle changes. Provides continuing education to patients and their family regarding treatment modalities, psycho-social adjustment to treatment, etc.
For Case Management: The Social Worker is responsible for providing direct care to hospital customers and working with the Patient Care Managers to assist in Discharge planning.
EDUCATION: Master’s degree in Social Work required, clinical experience in medical social work; CSW preferred.
EXPERIENCE: Social work experience in a dialysis setting working with health oriented community programs or satisfactory background in family casework with a total of at least two years’ experience. Additional experience in group work or teaching will be helpful raining aspects of the position.
