The Social Worker Intake Coordinator will educate, support and assess clients referred to the Chemical Dependency Program. The Social Worker Intake Coordinator will recommend treatment and coordinates admission to treatment for clients.

The Social Worker Intake Coordinator will respond to inquiries regarding to need for chemical dependency treatment from prospective patients, families and both internal and external referral sources. The Social Worker Intake Coordinator will utilize criteria to conduct clinical assessments in order to determine level of client functioning, appropriateness of treatment and type of treatment services required.

The Social Worker Intake Coordinator will arrange for admission by coordinating financial, clinical and other pertinent information including insurance verification and treatment authorization. The Social Worker Intake Coordinator will provide concise, relevant clinical information about clients to treatment team responsible for client care.

The ideal candidate for Social Worker Intake Coordinator is required to have Master's Degree with CA LCSW or LMFT licensure and CADAC Certification is preferred. The Social Worker Intake Coordinator should have at least 2 years of experience in Chemical Dependency.
