Employment is contingent on the continued availability of grant or non-general funds. Infant & Toddler Connection is a home visiting program based in the Lynchburg Health Dept. The person in this position will be expected to use their personal vehicle (with mileage reimbursement) for home visitations and other work related driving. A successful applicant for this position would join other counselors/service coordinators providing case management services for families with children birth to three years of age who have developmental or potential developmental delays. This counselor/service coordinator would complete/review developmental assessments, coordinate services and ensure that families are connected to all available community resources. Participation as a member of an interdisciplinary team, coordinating services with community agencies, interviewing families from a variety of backgrounds, evaluating the effectiveness of overall clinic services and following all federal and state mandates is a basic expectation.
Minimum Qualifications
Education and experience equivalent to BS/BA in human services, early childhood education, child development, or related field. Requires the ability to work with families of diverse backgrounds; must have effective oral and written communication skills; knowledge of family centered practices; knowledge of infant development, evaluation and assessment procedures
Preferred Qualifications
Knowledge of Part C Local System; knowledge of Part C regulations; knowledge of IDEA and/or the Infant & Toddler Connection program; Home visitation experience; Experience working with parents of young children.
Special Requirements
Employment is contingent upon satisfactory results of a state and federal criminal history background check and the Department of Social Service’s Child Abuse and Neglect Central Registry check, U.S. HHS IG Exclusion List check, employment reference check, and E-Verify. The selected candidate must complete a Statement of Personal Economic Interests as a condition of employment, if applicable (Va. Code § 2.2-3114). Other financial, credit, driving, or other background checks prior to employment may be required for certain positions.
VDH requires all Health-Care Personnel (HCP) to provide documentation of immunization or evidence of immunity to the following: hepatitis B, measles, mumps, rubella, and varicella; a one-time dose of tetanus, diphtheria, and pertussis (Tdap) vaccine regardless of the interval since the last dose of tetanus-diphtheria (Td) vaccine was administered (Td or Tdap boosters shall be received every 10 years); and the current year’s seasonal influenza vaccination.
Local and out of state travel including overnight stay, may be required from time to time to attend conferences, meetings, training, coverage, and professional development. May require some weekend and/or evening hours.
A valid State Driver’s License if required to operate a state vehicle or if operating a personal vehicle while conducting business on behalf of the agency.
