Job description
Assesses patients post discharge care needs and prepares the patient to transition from the hospital to the next appropriate care setting based on his/her individual needs. Uses understanding of medical conditions, insurance coverage and how physical/psychosocial injury may affect the patient once a patient leaves the medical setting to plan and initiate an individualized discharge plan.Performs a variety of tasks. A certain degree of creativity and latitude is required. Typically reports to a head of a department/unit. Typically requires certification or licensure in a specialty area with at least 2 years of related experience, but is not a registered nurse. Familiar with standard concepts, practices, and procedures within a particular field.
