Job description
Harmar Place is seeking a Social Worker to join our team! This position is responsible for facilitating communication and meeting needs of residents; protecting the rights of residents granted under federal statutes; and acting as liaison between residents, their families and United Church Homes (UCH), in order to provide a high quality of customer care and service, while upholding the policies and procedures of UCH.
Essential Functions Include
- Coordinates with other departments to create a comprehensive treatment plan for residents
- Communicates and establishes discharge and treatment goals, while coordinating with other agents/disciplines
- Advocates for residents, ensuring their rights granted under federal statues are protected
- Liaises between families and residents, and between residents and staff
- Tracks and manages mental health, well-being, cognitive loss of residents, using formal and informal processes
- Oversees room replacements/changes
- Maintains records and statistics on areas such as admissions, discharges and change statuses
- Interfaces with and trains staff on how to manage residents' behaviors/diagnoses
- Ensures all suspected abuse/mistreatment of residents or their families is reported to the proper authorities
Qualifications
- Bachelor's degree in Social Work/Services or related field (industry and state-specific)
- Must have two (2) to three (3) years of relevant work experience
- Must be able to proficiently operate a computer, the Internet, Microsoft Office and other relevant software programs, and basic office equipment
- Licensed Social Worker (LSW) or Master of Social Work (MSW) preferred
