Job Summary:
CareLinx by Sharecare and OnLok PACE will be partnering in recruiting Social Worker Aide. The Social Work Aide is responsible for providing support to the Social Workers in delivering services to OnLok participants.
Essential Job Functions:
- Assists the Social Workers in coordinating services for participants.
- Provides administrative and clerical support to the Social Worker as delegated. Duties will include tasks such as managing petty cash accounts, running errands, escorting participants, shopping, copying keys, trips to the Post Office, banks and assisted living facilities, providing assistance with translation, assisting with MediCal redetermination process, completing forms, surveys, transportation and other internal/external paperwork. Partner with caregivers and support system to develop care plan and deliver services.
- Being a companion, the Social Work Aide will spend quality time with participants in the day health center and encourage them to participate in conversation and other activities. The Social Work Aide will give reports and feedbacks to the Social Workers on a frequent basis.
- Other related duties as assigned.
Qualifications:
- Associate’s Degree in related field of human services.
- Ability to work independently and as part of multidisciplinary team
- Flexible and organized individual who takes initiative and is very detail-oriented
- Computer literate.
- Excellent written and verbal communication skills.
- Language requirement: Cantonese, Mandarin or Chinese.
- Ability to function under stress and with strong interpersonal and communication skills.
- Ability to function independently and effectively with shifting priorities and considerable pressure.
- While performing the duties of this job, the employee is regularly required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee occasionally is required to climb and stoop, kneel, crouch, or crawl.
- Specific vision abilities required by this job include close vision, and ability to adjust focus.
Work Environment:
- Working environment may be in an ambulatory clinic environment which is a space that is shared with other clinical staff which includes, but is not limited to, RNs, PCPs, LVNs, CNAs, and MAs).
- A working space is provided to conduct documentation and other administrative responsibilities.
- Work area is expected to be kept neat, orderly and clutter-free.
- Able to wear personal protective equipment (gloves, mask, gown, goggles, etc.) as needed.
- Required to work in a fast pace, highly distractible setting.
Job Types: Full-time, Part-time
Pay: $25.00 per hour
Schedule:
- Day shift
- Monday to Friday
- Night shift
- Weekend availability
