We are currently looking for a Social Services Assistant to join our management team here at Linda Mar Rehabilitation.
Position Responsibilities:
- Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility.
- Participate in discharge planning, development, and implementation of social care plans, and resident assessments.
- Interview resident/families as necessary and in a private setting.
- Involvement with the medical, social, and emotional needs of the facility's residents.
- Document the social needs of each resident
- Prepare for the discharge of the resident (Home Health, DME, etc).
Requirements:
- At least one year of experience in social services work in a healthcare related role.
