POSITION OVERVIEW*
HomeRise seeks a creative, engaging, and caring professional to oversee programming for Transition Aged Youth (TAY) that were formerly experiencing homelessness in San Francisco. Supervised by the Assistant Program Director and working in conjunction with the site-based Resident Services Team Lead, the Senior Case Manager (SCM) is responsible for providing services to our residents. The SCM follows the principles of harm reduction and trauma-informed care to engage a caseload of about 15 TAY participants/residents in service connection, groups and activities focused on community building, education, or skill development, crisis intervention, and housing retention. All services are provided within the context of harm reduction and within a Housing First model.

ESSENTIAL FUNCTIONS*

Provide housing/program retention services.
- Provide case management services of up to 15 residents.
- Create and track service plans.
- Provide activities for community building and empowerment.
- Provide services using harm reduction principles.
- Conduct intakes and ensure that appropriate follow-up and/or referrals are complete and documented.
- Provide ongoing creative outreach and engagement for supportive counseling, referral, and involvement in building and community events.
- Provide crisis intervention, conflict resolution and de-escalation services.
- Assist residents in preparation for unit inspections and/or annual re-certifications.
- Participate in team meetings, staff meetings, department meetings and organizational work groups.
- Accompany residents to community based appointments.
- Develop and facilitate groups and community building activities.
- Provide back-up coverage for other Resident Services staff.
- Other duties as assigned.

Location: 374 5th Street, San Francisco, CA
- Status: Full-Time / Non-Exempt
- Pay Rate: $26.00/Hr. (bilingual premium + $0.50/hr.)
- Schedule: Tuesday through Saturday 11 AM - 7 PM or Sunday through Thursday 9 AM - 5 PM
- Union: This position is under the collective bargaining agreement with SEIU 1021
- Reports to: Assistant Programs Director
QUALIFICATIONS

High School diploma or GED required.
- Bachelors degree in a behavioral science (social work, psychology, clinical psychology) or a social service certification (CAADAC, CCAPP, etc.) or equivalent experience preferred.
- Two years of experience in the provision of social services or equivalent experience. Demonstrated ability to work with homeless or formerly homeless individuals and those struggling with substance use and mental health issues and disabilities.
- Ability to provide effective services to diverse populations.
- Ability to work collaboratively with referring providers in San Francisco.
- Experience in crisis intervention, conflict resolution, and counseling.
- Experience in organizing groups, group facilitation and group development.
- Demonstrated commitment to help break the cycle of homelessness and poverty, and to empower formerly homeless people by involving them in all aspects of HomeRise.
- Strong commitment to doing social justice advocacy.
- Administrative and computer skills (MS Word, Excel).
- Desired qualification: Fluency in Spanish/Mandarin/Cantonese/Russian a plus.
