SUMMARY
Parsons & Associates has been engaged by a growing hospice agency in the greater Apple Valley area of California to recruit a clinical RN Case Manager. The hiring company is an Equal Employment Opportunity Employer and does not discriminate on the basis of any protected class or veteran status.
The Case Manager provides coordination of all hospice services provided to patients/ families by the interdisciplinary team, in all settings by serving as a liaison between patient, family and all disciplines involved in the provision of care and services. Assures quality, appropriateness, and continuity of care. Supervises patient care provided by LVN and CHHA routinely. The Case Manager sees patients in the field and manages his/her patient caseload.
DUTIES:
1. Carries out administrative and patient care duties as directed.
2. Abides by Agency policies and procedures and interprets them to personnel, patients, medical staff and public.
3. Performs administrative duties: Gives and receives report, answers telephone inquiries, takes and transcribes physician orders. Updates care plans.
4. Performs on-call duties as assigned.
5. Assists Hospice Director of Patient Care Services with orientation and instruction of other nursing personnel.
6. Participates as a member of the IDT
a. Attends IDT meetings.
b. Arranges additional IDT conferences as needed.
c. Presents patient's plan of care at IDT meeting.
7. Evaluates patient condition and patient/family needs, provides comprehensive assessment, and initiates Hospice Plan of Care with Hospice Medical Director and other members of the IDT.
8. Admits and orients new patients/families.
a. Checks identification.
b. Checks physician orders, transfer and preadmission information.
c. Reviews patient medical history and patient/ family social history.
d. Develops and initiates individualized patient care plans.
9. Integrates hospice and facility Plan of Care and care plans.
10. Assembles hospice section of facility charts,
a. Checks to see that consents for hospice, election statements and other required forms are attached.
11. Periodically reviews charts for completeness and correctness.
12. Orders prescribed medications, requisitions supplies and equipment.
13. Provides nursing assessment, plans and provides interventions and evaluates outcomes for patients in all settings.
14. Monitors equipment for performance and usefulness.
15. Maintains medical records.
16. Documents care given.
17. Participates in QA/QI Program.
18. Takes reports of unusual occurrences, work place hazards, etc, from subordinate employees.
19. Participates in and promotes, injury and accident prevention.
20. Participates in the evaluation process of employees under his/her supervision.
21. Practices and promotes Infection Control according to policy and procedure.
22. Reports all hazards to health and safety to supervisor.
23. Attends all scheduled meetings on time and prepared for the meeting
24. Maintains confidentiality and protects PHI.
25. Abides by all company policies and procedures.
26. Other duties as assigned or delegated by the Director of Patient Care Services or Administrators.
RESPONSIBILITY:
1. Provide professional nursing service to patients, following established policies and procedures, and standards of care as outlined in clinical policy and procedures.
2. Provide quality nursing care.
3. Provides professional nursing services without regard to race, culture, or ethnicity, spiritual beliefs, sex, age, socioeconomic background, or disease process of the patient/ family.
4. Attends all required/ mandatory meetings: Licensed Staff, All Staff, IDT and others as assigned:
a. Is on time for meetings, barring emergency patient needs.
b. Is prepared for all meetings.
c. Participates at a professional level in all meetings as needed or assigned.
5. Accuracy and promptness in carrying out physician's orders.
6. Effective and efficient use and care of supplies and equipment.
7. Maintains productivity and organization of workflow.
8. Maintain accurate patient records, documentation. Completes all paperwork and forms and meets paperwork deadlines.
9. Cooperative and congenial with medical records in accomplishing submissions, corrections, completions and addendums to the medical record in a timely manner.
10. Cooperation with other personnel.
11. Maintain confidentiality and protect PHI.
12. Demonstrates ability to function independently with minimal supervision.
13. Makes comprehensive nursing assessments and participates in determining need for skilled nursing, SW, Pastoral Counselor, and Volunteer.
14. Consults with attending MD regularly re: changes in the patient's condition; implements appropriate changes in the Plan of Care. ยท
15. Recognizes when patient's visit schedule should be altered based on change of condition.
16. Instructs, counsels, supports patients/ families.
17. Promotes attainment of patient/family goals.
18. Promote and protect patient/family autonomy, independence and dignity.
19. Provides in-services to educate and train staff to promote the knowledge and skills needed to care for hospice patients according to accepted hospice goals and principles.
20. Report all suspected abuse or neglect to appropriate agency.
21. Facilitates communication among all IDT members.
22. Other duties as assigned by supervisor or Director of Patient Care.
23. Available and willing during working hours to take patient visits as assigned.
24. Seeks assistance and advice from team members and supervisors when needed.
WORKING CONDITIONS:
Well-lighted, well-ventilated rooms and halls. Sits, stands and walks most of workday.
Lifts, moves, supports patients and equipment with assistance.
Exposed to infectious diseases, odors, blood, body fluids and secretions, wound drainage, feces, urine, and all other substances that may be present in working with the ill, infirm and elderly.
Must deal with patients and public on personal basis under all conditions.
Must handle emergency situations, give emergency care and give reassurance to patients/families.
Requirements:
Graduate of an approved school of nursing. Current RN license in the State of California.
At least twelve months experience in Hospice, medical-surgical nursing, home health, Skilled nursing or community health experience essential.
Good physical and mental health.
Must have sincere desire to work with the aged and infirm.
Must have thorough knowledge of medical, social, and psychological needs of patients. Must be willing to perform non-professional duties.
Must be able to supervise and instruct others.
Must be well-groomed, courteous, tactful, patient, kind and pleasant when dealing with patients. Must be level headed in emergency situations. Must have acute sense of responsibility.
Must possess a spirit of cooperation and enthusiasm in order to create an atmosphere conducive to rehabilitation and growth.
Ability to manage personal stress.
Adaptability to changing circumstances, schedules, assignments, and patient needs.
Current California Driver's License and insurance is required. Yearly Motor Vehicle Record print out from the DMV.

Pay: $46.00 - $50.00 per hour
