Position: Registered Nurse (RN) ACT Team- Per Diem
Hours: Weekdays Only
Rate of Pay:
$55
Position Summary: The RN will assist The Guidance Center of Westchester’s mobile ACT team to provide comprehensive recovery services to adults with serious mental illness throughout Westchester County.
Responsibilities:
Support a high-performing culture aligned with TGCW core values and B.A.S.I.C Tenets: Balance, Accountability, Satisfaction, Inclusion, and Communication.
Conduct home and community visits.
Complete health assessments.
Administer injections and instruct participants on medication adherence.
Coordinate laboratory services and reports.
Collaborate with psychiatrist, registered nurse, clinicians, and other team members.
Transports and utilizes required medical equipment and supplies weighing approximately 25 – 30 lbs. to and from patient homes/care facilities, TGCW offices and other locations.
Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
Document notes in electronic health reporting system.
Order and store medical supplies.
Attend clinical and administrative team meetings.
Complete required staff development trainings.
Qualifications:
Current NYS Registered Practical Nurse.
Minimum 2 years’ experience with adult psychiatric treatment preferred.
Electronic health record experience required.
Clean and current NYS driver’s license.
Travel Requirements:
~80%
