We are immediately seeking a Resident Care Managers RN to IMMEDIATELY join our amazing Nursing team!

Includes a $15,000 signing bonus!

Hours: Full-Time, 9am-5pm

Transitional Care Unit RCM Job responsibilities:

• Medicare /PPS meeting weekly

• Performing weekly notes / summary tcu updates for all Medicare

• Pdpm meeting MWF with mds.

• Reviewing Medicare certification/ recertification’s

• Completion of unit Incident reports

• Attending in Morning huddle with providers

• Review of rehospitalization

• Attending in 48 hour care plan meeting with new admissions and family

• Management of day to day operations of TCU nurses ,nacs

• Management of high alert family

• Infection control rounds

• Management/performing audits of cleanliness environment (med rooms/ soiled utilities / clean utility) med carts.

• Over seeing discharge process, teaching, etc

• TCU Nursing orientation

• Attending in Staffing meeting

• (Should be part of role- chart review)

• 
  Develop process/tools to smooth operations such as “How to prevent rehospitalization.

• 
  Weekly inservice with staff to discuss pressing issues regarding the unit skin, falls, high alert family, behaviors, high acuity.

• Overseeing follow up appointment of residents
• Overseeing Unit Clerks
  
• Following up on missing Medicare documentations and overdue UDA

Job requirements:

• Active WA state RN license
• CPR certification
• Excellent interpersonal skills
• Able to work with time constraints and stress
