This is a traveling position with high autonomy! We have an invested clinical management team that ensures you always have support! We promise a comfortable case load with flexible scheduling (make your own schedule) to help with work life balance.

• $39-$41/hr, predicated on experience
• Full-Time is GUARANTEED 32 hours minimum
• Travel Reimbursement (from your home to client, from client to client, from client back home)

RN Case Manager Requirements:

• Must be Active on the MN Board of Nursing
• Must be current on CPR Certification
• Minimum of one year of experience as a Nurse or two years’ experience as a paramedic with EMS, preferred but not required
• Must be able to travel at least up to 20 miles from where you live
• Must have Driver’s License with insured, reliable transportation

RN Case Manager Essential Duties:

• Provide in-home care for clients based on personalized plan of care
• Oversee caregivers and perform supervisory visits and competency testing
• Perform and maintain on-going client case management
• Thoroughly document assessments and cares using eCharting system
• Provide care coordination for individual clients, including coordination with other agencies, healthcare providers, pharmacies, medical supply companies, etc
• Manage the client’s nursing schedule; be prepared to fill shifts urgently if needed
• Other duties assigned or delegated by DON, including computer work in the office if necessary
