Sangre de Cristo Community Care is looking for Per Request Needed Registered Nurse to join our dynamic home health team in La Junta, CO. Serving a variety of patients across Southern Colorado, including pediatric patients and those with developmental disabilities, Sangre de Cristo Community Care is known for delivering exceptional high-quality compassionate care with a strong team of local health care professionals, including skilled nursing, physical therapy and occupational therapy, providing a seamless opportunity for high-quality hospice, palliative care and home health services to residents in southern Colorado.

Responsibilities:
- Plans, coordinates, implements, and evaluates the nursing care of all patients.
- Utilizes clinical knowledge and skills to identify patient needs and coordinate services and referrals to meet these needs
- Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.
- Must be at least 21 years of age.
- A valid non-restricted RN license in the state of Colorado is required.
- Must have a valid non-restricted driver’s license.
- Flexibility and availability to work a weekend schedule if needed.
- At least 1 year of home health experience preferred.
- Participation in the seasonal Coronavirus Disease 2019 (COVID-19) Vaccination program is a requirement for all Sangre employees
