The Registered Nurse (RN) Case Manager is responsible for ensuring clients are provided proper care by caregivers; The RN will assess, plan, implement, and evaluate client(s) in order to maintain the client’s health and safety while at home. The RN will also provide guidance and training to caregivers to maintain proper services to maintain appropriate, and consistent manner within the scope of their license.

Schedule:
Office duties once a week; eight (8) hours.

Field duties twice, or more days, during the week, to complete assessments.

A minimum of ten (10) client assessments are to be completed in one week.

Essential Functions:
Creates and maintains the POC (Plan of Care), this includes a re-evaluation of the patient as necessary and making necessary adjustments.

Update client Excel rosters weekly and communicate frequently with Care Coordinator Supervisor and QA Admin regarding rosters and appointments.

Scan and file client and caregiver assessments.?

Make appointments with clients and caregivers.

Communicating with State nurse monitors on requested medical documents.

Full completion of the OASIS assessment and any supporting documentation per assigned patient.

Facilitates timely discharge based on the client’s specific goals.

Educates and instructs the patient, patient’s family, and other staff as required.

Supervises and teaches LPN’s, CNA’s, HHA’s as required for patient care according to law and regulations.

Notifies the patient’s physician and staff responsible for significant changes in a timely manner.

Coordinates all services provided; consult with other health professionals as needed.

Effectively plans, organizes, sets priorities, and completes work assignments with minimum direct supervision per agency policies, within established time frames.

Verbalizes knowledge and purpose of patient rights and responsibilities.

Complies with Touching Angels’ policies and procedures.

Update appropriate systems and inform Care Coordinator Supervisor of any urgent matters flagged by the RN during their visit.

Submit weekly RN excel tracking report to supervisor and use LTSS/ISAS system to review client’s POS and SNF information. ??

Minimum Requirements:
Education: A formal nursing education at a college, university, or hospital that presents a certificate in the form of a bachelor’s degree, associate degree, or other.

Licensing: Active and unencumbered RN licensure in MD/ Compact States

Maintain CPR/BCLS

Physical Demands: Stooping, turning, bending, squatting, kneeling and the ability to lift to 50 pounds; constant/repetitive standing; requires normal, correctable vision and hearing, and the ability to accurately discern color as necessary to perform job functions??

Health clearance – Annual Physical, TB, Hep B?

Be able to operate a Multifunction copier (print, scan, fax, copy) Email and Text.

At least 2 years of pediatric, home healthcare, or hospital experience in the past 2 years.?

Must be authorized to work in the United States.

Must undergo a background check.

Must be vaccinated for Covid 19.
