Job as a Recovery Coach:
The Peer Recovery Coach works with the care management team in providing a seamless continuum of care. Maintains regular and constant communication with patients to provide a set of non-clinical activities that engage, educate and offer support to patients. Will be responsible for coordinating care and serve as patient advocate. The Peer Recovery Coach will help patients to remove personal and environmental obstacles to health care access and help them remain in recovery.

Hours are part-time 7 AM - 12:15 PM for 3 days per week, equal to about 16 hours per week. Days will vary.


Responsibilities as a Recovery Coach:

  • Facilitate peer lead support groups.
  • Serve as a mentor for patients by demonstrating ways to become empowered and self-responsible.
  • Providing care management and care coordination services for patients in the clinic. Responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements;
  • May also be responsible for conducting individual, and family educational sessions, various duties related to coordinating treatment and discharge activities, as needed;
  • Coordinates in a timely manner, issues or activities relevant to the team, and/or to the team’s relationship within the facility or treatment milieu; and,
  • Perform other duties as assigned
Qualifications:

Skills/Qualifications as a Recovery Coach:

  • High School Diploma preferred, but not required
  • 1 year of experience working in behavioral health or substance use disorder recovery setting preferred
Licenses/Certifications:
  • Certified Intentional Peer Support Specialist (CIPSS) or CCAR Peer Recovery Coach. AHCTC
