PATH is seeking candidates with experience working with vulnerable populations to join our San Jose Outreach team as a Rapid Resolution Specialist. This position welcomes and encourages candidates with lived experience related to homelessness to apply.

ABOUT OUR OUTREACH PROGRAMS
Our Street Outreach programs work to engage individuals who are currently homeless and living on the streets in an effort to provide and link them with services that will ultimately lead them to stabilize in housing long-term.

WHAT IS A RAPID RESOLUTION SPECIALIST?
As part of the Outreach team, the Rapid Resolution Specialist serves as one of the first points of contact for clients who are experiencing first time or short-term homelessness in the San Jose community. Services provided by the Rapid Resolution Specialist are designed to identify rapid solutions to preventing or ending homelessness for participants. Responsibilities of the Rapid Resolution Specialist include:

  • Conducting daily street outreach to develop relationships and build rapport with clients in the interest of helping them address their barriers to obtaining services, housing and healthcare
  • Conducting screening for newly homeless individuals and families
  • Facilitating problem-solving sessions aimed at assessing clients' existing supports and resources that can aid in the quick resolution of homelessness
  • Assisting with housing applications, the completion of supportive and subsidized housing paperwork, surveying the rental market for and identifying affordable permanent housing options, and advocacy with prospective landlords
  • Collaborating with clients to compile a detailed household budget and determine what level of funding is necessary for the family top be successfully housed
  • Facilitating one-time financial assistance to aid families in stabilizing in permanent housing placements
  • Providing information, referrals, linkages, and advocacy for all other identified needs

WHAT YOU BRING
We're looking for candidates that:

  • Have lived experience with issues related to homelessness
  • Demonstrate knowledge of and/or experience with evidence-based service delivery practices including:
    • Critical Time Intervention
    • Harm Reduction strategies
    • Non-violent crisis intervention techniques
    • Motivational Interviewing
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment

MANDATORY REQUIREMENTS
For this role the Rapid Resolution Specialist must:

  • Have at least 2 years of experience working with highly vulnerable populations
  • Hold a Bachelor's Degree in a related field OR have a comparable combination of education and/or experience
  • Be able and willing to work flexible hours which may include evenings or weekends
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

  • Have reliable transportation
  • A valid driver's license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.

FLEXIBLE WORK
PATH is a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
