QUALITY ASSURANCE, SCHEDULING & COORDINATION OF CARE SPECIALIST: HOME HEALTH & HOSPICE CARE (QASC)
Responsible to: CEO, COO, DON, ADON, Administrator
QASC SPECIALIST Reviews clinician documentation for accuracy. Communicates regularly with clinical staff and provides ongoing education. Assists the CEO, COO, DON, ADON and Administrator for Quality Improvement in implementing agency’s Quality Assurance and Improvement Programs.
JOB DUTIES/KNOWLEDGE:
- Monitors client clinical records concurrently and retrospectively, utilizing established outcome criteria (i.e., plan of care, goal, and objectives).
- Assists clinicians in accurate coding, OASIS, and other required documentation completion.
- Reviews the nursing charts for all visits (SOC, RECERT, ROC, Discharges, Supervisory Visits, Skilled Nursing visits, and so on) and provides an update to the team during administrative meetings, IDG meeting, Nurses' case conferences.
- Coordinates with the DON and ADON to conduct quarterly QAPI meetings for home health and hospice.
- Analyze, review and report on nursing charts, charting trends, and quality indicators.
- Updates and coordinates with the supervising nurse and external clinical staff regarding changes in the patients' treatment plans when necessary.
- Monitors compliance to policies and procedures including but not limited to clinical documentation.
- Resolve charting issues and recommend changes to procedures.
- Update corrections on documentation after retrieval from appointed nurse(s).
- Assist the quality department with surveys and audits as assigned.
- Assist with professional education activities to encourage professional skill proficiency for all agency clients.
- Participate as a consultant in the implementation of chart review tools, standards of care and outcome measures.
- Required to identify clinical problem areas and inform/train staff in the correction of any deficiencies.
- Assist with quality assurance and policy compliance training programs and develop reference materials to resolve inadequacies.
- Develops, implements, and maintains systems to ensure effective and high-quality professional nursing care.
- Maintains confidentiality of all employee/client records and information.
- Acts as a coordinator, facilitator or liaison to between the administration and the clinical staff.
- Participation in performance improvement projects to increase high quality, cost-effective methods of practice.
- Prepares meeting minutes for IDG meetings, Nurses' case conferences and QAPI meetings.
- Provides coordination of care, in conjunction with the intake coordinator, starting right from when the agency receives a referral.
- Reviews the referral clinical documentation, along with the nursing team, to make sure the respective patient is eligible to be admitted to home health/hospice care.
- Coordinates with the nursing team as and when they have clinical coordination needs throughout the patient care process.
- Prepares the weekly and on-call schedule by COB, Thursday, and shares with the nursing team.
- Attends the daily morning and end-of-the-day meetings with the COO.
- Assists with orientation of new staff, as necessary.
- Organizes and/or participates in scheduled staff meetings and in-service education, client care conferences as needed.
- Support the leadership with process development and oversight.
- Other duties as assigned by the leadership.
- QASC specialists with a license to practice as a registered nurse/licensed vocational nurse will:
a. Act as a clinical and regulatory resource to the CEO, COO, DON, ADON and Administrator.
b. Maintains clinical skills and function in the field as a field clinician and support the on-call clinician.
c. Acts as supervisor on-call as necessary.

QUALIFICATIONS
1. Licensed Vocational Nurse or Registered Nurse
2. 1 year of QA experience is required.
3. Home Health and Hospice QA preferred.
Job Type: Full-time
Pay: $40,000.00 - $47,000.00 per year
