Sinai Urban Health Institute (SUHI) is hiring a Project Manager. In partnership with the Operations Manager, this position is responsible for the operations of the Community Health Worker (CHW) care model for the South Side Healthy Community Organization (SSHCO), which comprises 13 healthcare organizations including safety net hospitals, health systems and Federally Qualified Health Centers. The model will better connect existing health organizations, increase access to care, address some of the most challenging health issues, and ultimately, build health equity and ensure stronger, healthier communities across the South Side of Chicago.

The Project Manager is responsible for managing CHW workflow development, implementation, and execution, understanding the sites’ training, technical, and HR requirements to successfully onboard CHWs, and ongoing operational support of the coalition sites and CHW staff as it relates to the CHW care model. The position is grant funded.

WHAT YOU WILL DO:

• Document existing care coordination, patient, and clinical workflows at SSHCO coalition site

• Collaborate with coalition site staff to determine the training and human resource requirements for CHWs to work at each coalition site (e.g., EMR training, site orientation, vaccination requirements, ID badge, etc.).

• Collaborate with coalition site staff to customize CHW workflow to integrate into existing operations.

• Implement CHW workflows, processes, and protocols in SSHCO participating hospitals, clinics, and CBOs.

• Coordinate and participate in interview, hiring and onboarding process for CHW staff recruitment.

• Develop and deliver onboarding program for CHW hires and acclimating them to their roles.

• Provide direct oversight and management of CHW site supervisors.

• Support technology enablement (e.g., EMR, Unite Us, new tools).

WHAT YOU WILL NEED:

• Bachelor’s or Master’s degree in public health, business or health care administration, non-profit management or a related field.

• Two to three years’ experience working in a project management, practice manager or other operational role at a clinic or hospital.

• Experience overseeing a CHW, population health management, care management or similar program, or related healthcare consultant role, preferred.

• Strong business acumen and project management experience.

• Ability to effectively manage change and shift gears as project needs evolve.

• Strong verbal and written presentation and communication skills.

• Demonstrated commitment to racial and health equity.

• Ability to supervise staff across multiple sites in the completion of projects within budget and time.
