Job description
We’re actively recruiting a Program Specialist for our growing “startup within a startup,” Aledade Care Solutions (ACS). Aledade Care Solutions is a 150-employee wholly-owned subsidiary of Aledade, Inc., itself a 1000-employee national leader in value-based care. Our mission at Aledade Care Solutions is to build a next generation care delivery business, serving patients across the country with new and innovative virtual care models — and to do that, we need YOU!
This role will report to the Senior Director on the ACS Innovation Team, with close exposure to many other senior leaders. The Program Specialist will be a part of a newly established program management function to support the design, launch, test and scale up of new clinical programs for ACO patients. . ACS provides a suite of clinical programs, with each focusing on a type of patient or type of care. This role will initially focus on developing programs in a top priority area for ACS: Improving healthcare for Aledade’s high cost/high need (HCHN) patients. This role’s focus will likely include scaling nascent ACS programs targeting HCHN patients (including our new interdisciplinary care management program), and developing new programs targeting HCHN populations (likely including a virtual behavioral health program and/or coordinating patient care transitions with Skilled Nursing Facilities).
We are flexible with respect to geographic location, and the ideal candidate will be comfortable working remotely/work from home within the US or from our offices in Durham, NC; New York, NY; Austin, TX; or Bethesda, MD.

As a Program Specialist you will:

Lead the design and execution of new programs. We are building new programs and designing partnerships from scratch at ACS, which means this role will include innovation, incubation, and development of new programs:

    • Manage the financial analysis to determine the economic model for new programs, and to shape rollout plan (i.e., phasing of markets, ACOs, and practices)
    • Manage the ongoing vendor relationship(s) including supporting defining partnership scope and term sheet, contracting, and operational rollout of program
    • Advance conversations on the right go-to-market approach for any program you own, whether buying an asset, building in house, or partnering with another company
    • Shape the HCHN Program Strategy, and the way in which ACS/Aledade execute and scale pilots into transformation initiatives available across the Aledade network
    • Use scalable processes and approaches that enable others in the PMO

Own project management for one or more programs. You will be the leadership’s team key point person for one or more programs and will be expected to:

    • Own the cadence of meetings and deliverables for the team, including setting priorities and deadlines, and tracking against a broader project plan.
    • “Manage up,” flagging when programs face major risks or challenges that need attention, resources, or other mitigation steps.
    • Communicate with a broad range of stakeholders on a regular basis, including members of the program team as well as senior management of ACS and Aledade, adapting your voice and content as appropriate for each audience.
    • Manage at least one business analyst to execute the program
    • Become a SME on addressing needs in our HCHN patient population for the rest of Aledade

Lead cross-functional team to define and execute end-to-end program. You will lead primarily through influence rather than direct authority, working with teams across the organization (both at Aledade, Inc. and ACS) including:

    • Work with and through the practice engagement team to ensure that new and existing programs are built in a way that complements Aledade practices’ clinical models and to ensure uptake
    • Work with performance team and network success to ensure field team is effectively prepared to rollout program to our practices
    • Designing dashboards and metrics for tracking program performance with the business intelligence team
    • Defining algorithms for patient identification and prioritization with the impact analytics team
    • Ensuring legal compliance and a high ethical standard in partnership with the legal and compliance teams
    • Work closely with with the product team to define the patient (and as relevant, practice) experience, and develop the product that enables that experience
    • Work across stakeholders to develop a proof of concept (i.e., pilot)
    • As relevant, work with external partners to deliver program
    • Participate in “other duties as assigned.” We’re a startup and we can never predict exactly what we’ll need. That’s part of the fun of the work — you should expect that some percentage of your work will evolve as the Company’s needs evolve.

What We Value

    • 3+ years of professional experience, possibly in consulting or project management roles, with experience executing a project end to end
    • Experience working directly with clinicians and patients (especially HCHN), preferably in value-based care setting
    • Experience managing direct reports and working with cross-functional teams
    • Advanced facility with common office software tools (such as Google Suite) and ability to quickly learn others (such as Monday.com)
    • Strong verbal and written communication skills
    • Strong strategic planning and analytical skills
    • Poise, confidence, and maturity interacting with key stakeholders
    • Ability to build strong relationships and work with individuals at all levels of the organization
    • Highly organized and detail-oriented in planning and executing tasks
    • Highly efficient in managing multiple concurrent diverse tasks and projects
    • Ability to use independent judgment and work under pressure on competing responsibilities with resilience, and a high level of ownership
    • Given that we work across different time zones, some non-traditional hours for early or later calls are an expectation of this role.
    • Bachelor’s degree
    • Program or Project management certification preferred
    • Thinks beyond their immediate team and contributes to making Aledade holistically better (active engagement in D&I efforts, culture initiatives, facilitating training, leading roundtables, etc.)
All prospective hires will be required to demonstrate that they have been fully vaccinated, including booster shots, against COVID-19 with a COVID-19 vaccine for which the U.S. Food and Drug Administration has issued a license or an Emergency Use Authorization prior to mutually-agreed upon start date at Aledade, unless they qualify for a medical or religious accommodation to this vaccination requirement. In certain limited circumstances, Aledade will accept documented proof of prior infection with COVID-19 in lieu of a booster shot.
