TITLE: Program Coordinator (CMHC)

REPORTS TO: Program Director CMHC

DEPARTMENT: Community Mental Health Centers

SUMMARY: Provides overall management of the Community Mental Health Centers (CMHC) planning effort. Work with CMHC Project Director to meet CMHC expectations and goals. This staff member will oversee and assure continuity of care from in-patient hospital discharge to either routine outpatient care or a higher level of care, among other duties. The position requires a LCSW, trained in DBT and CBT, work experience in group and individual psychotherapy and assessing/treating patients with dual diagnosis preferred.

DUTIES:

  • Works closely with Director of Clinical Services and CMHC Director to perform the following duties:
  • Assign clinical responsibilities to staff
  • Manage program caseload
  • Monitor and coordinate transitions of care
  • Provide administrative oversight to staff within the CMHC and ensure all workflows and compliance expectations are being met.
  • Ensure target populations have access to needed services according to contractual requirements and Agency Mission.
  • Work with Program Director toward implementing agency trainings that meet grant expectations.
  • Monitor program adherence and progress toward meeting Agency, funding source and regulatory requirements. Including but not limited to levels of service expectations and ensuring the agencies mission is understood, fostered and supported throughout the program levels.
  • Perform other work-related duties as assigned.
  • Available to travel and work evening and weekend hours as required by agency needs.
  • Motivate staff to complete all tasks assigned and to provide clients with wrap around services.
  • Develop and promote a positive work environment that is supportive of staff and agency mission.

EDUCATION REQUIREMENTS/QUALIFICATIONS:

  • Licensed Clinical Social Worker (LCSW)
  • Two years of related experience
  • Strong leadership skills and ability to motivate others.
  • Trained in DBT or CBT evidence based practices.

QUALIFICATIONS:

  • Superior attention to detail, research, organizational and problem solving skills
  • Ability to work independently and as part of a team
  • Excellent communications skills, both in-person and in-writing
  • Tech savvy with experience with Mac OS, Google Docs, MS Office and other web-based applications
  • Thrive in a fast-paced and dynamic environment
