We are looking for an experienced full time Population Health Coordinator for our pediatric offices in Frederick. Under the direct supervision of the Operations Manager, this position is an integral position that works with the clinic care team to proactively track the care needs of the patients on their panel and to ensure that patients receive the services they need to optimize their health and well-being. The diverse responsibilities can include, but are not limited to: encouraging patient engagement through direct contact with patients and their families, tracking and closing gaps in care, assuring accurate patient panels, coordination with health plans, and assimilation of multiple data sources to ensure an accurate picture of the quality of care of the patient population. This position is a job share with the front office team, so some front office duties will be expected. The anticipated hours are Monday through Friday between 9am and 5pm with some flexibility to work later as needed. The salary range for this position starts at $45k depending on experience. We offer a $1000 hiring bonus payable upon successful completion of six months of employment.
Requirements:ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Lead the population management process by actively monitoring and sharing patient and population quality data with the care team in order to facilitate improvement in care outcomes
- Understand and champion proactive care for the entire population of patients connected with The Pediatric Center of Frederick, not just those who come in for visits
- Maintain in depth knowledge of all quality metrics for which the organization is held accountable
- Proactively contact families/patients identified for health maintenance and other gaps in care
- Engage with families/patients through routine outreach utilizing phone contact and email as needed
- Identify and decrease the impact of barriers interfering with a patient’s access to health care
- Actively maintain the provider panel section of the clinic database
- Ensure registration of new patients with provider attribution
- Maintain confidentiality of sensitive patient information at all times.
- Properly utilize reporting database and manipulate reports to obtain actionable data
- Display high standards of office conduct

EDUCATION, QUALIFICATION AND CREDENTIALS:
- High school education or equivalent
- Experience in a clinical setting required
- Experience with panel management and quality metrics preferred
- Experience with Electronic Health Records require
- Knowledge and understanding of medical terminology and clinical procedures.
- Experience in customer service, computers, general office procedures, multitasking, and understanding of insurance terms
SKILLS, KNOWLEDGE AND ABILITIES:
- Strong professional interpersonal, communication, telephone, customer service, time-management, and decision-making skills required
- Fluent English
- Excellent relationship-building and influencing skills
- Understanding of insurance benefits
- Ability to pay close attention to details
- Demonstrated proficiency working with an EHR system
