Position Summary
The Pharmacy Technician will be responsible for assisting in medication coordination and medication management duties for all identified patients, specific to kidney health management. The Pharmacy Technician will be available to assist incoming callers who have pharmacy related questions. Daily activities will include maintaining individual metrics, engagement of members into the medication management program, managing a set caseload of mixed acuity members, reviewing and/or obtaining member data, data entry in HealthMap’s documentation system, updates of identified medication problems, barriers, interventions, and goals, patient/member teaching, and assistance with ongoing case management.

Responsibilities
  • Receive inbound calls, offer help to patient questions, and provide medication therapy management (MTM) services as needed with pharmacist oversight.
  • Receives and responds to calls from both provider/physicians’ offices and pharmacies.
  • Makes outbound calls to physicians, pharmacies, and offering medication management services to patients.
  • Contacts patients to explain the MTM program and schedules calls for pharmacists to complete patient interviews.
  • Works with customer service inbound phone calls and voicemail messages, determines appropriate action and escalates calls as needed.
  • Evaluate medications that are high-cost, high-risk or high-volume.
  • Conduct education for providers and staff regarding new drugs, new indications, medication opportunities, and development of new medication interventions.
  • Participate in the development of treatment algorithms, guidelines, policies and procedures and other kidney disease management efforts
  • Create processes to address and improve medication adherence.
  • Adhere to policies, procedures and state and federal regulations to ensure compliance and confidentiality of Protected Health Information (PHI). Participation in Compliance and other important training is a condition of employment.
  • Performs other duties as assigned.
Requirements
  • Familiar with medical insurance terminology and processes
  • Familiar with general pharmacy terminology (SIG codes, brand, and generic names of medications)
  • Successful completion of a Certified Pharmacy Technician Course and pass certification exam (state or national)
  • Three (3) years' experience in managed care pharmacy industry required - 1-3 years of managed care; or 3 years min in retail pharmacy
  • Comfortable processing information and making decisions quickly
  • High school diploma or equivalent
  • Must be proficient in Microsoft Office: Outlook, Word, Excel, PowerPoint
  • Access to a reliable computer with audio/video capabilities and a high-speed internet of at least 100 Mbps
