Job Description
Physician Orders
- Review and interpret physician orders for appropriateness.
- Dispense prescriptions to patients in accordance with hospital policy and state and federal laws.
- Maintains an up to date patient profile ensuring an accurate eMAR.
- Reviews drug orders for incompatibilities, interactions, and contraindications.
Drug Compounding/Delivery
- Verifies the work performed by the technical staff including data entry, cassette filling, unit dose drug distribution, and IV compounding.
- Supervises the pre-packing and required recordkeeping of pharmaceutical products.
- Reviews controlled drug delivery requests by the pharm tech to ensure accuracy and proper record keeping.
- Prepares IV solutions for administration via the intravenous route and ensures the sterility/stability of these solutions.
- Compounds chemotherapy drugs for the oncology infusion center. Double checks the work of the compounding pharmacist or technician.
- Verifies the dose, administration parameters, and indication for all oncology regimens.
- Reviews chemotherapy drugs including dose, administration, indication and safety.
- Participates in clinical programs within the department such as; drug level monitoring, antibiotic monitoring and pharmacokinetic consults.
- Complete admission medication reconciliation for hospital inpatients on an as needed basis.
- Maintains up to date knowledge of the profession by participating in organizational memberships. This may include attending meetings, lectures, in-services, or seminars.
- Provides drug information (dosing, compatibilities, and formulary policies) to physicians, nurses, and other health care practitioners
- Responds to emergencies (i.e.- Code Blue) within facility.
- Performs other duties as requested.
- Participates in departmental or organizational meetings and trainings.
- Demonstrates ability to use equipment relevant to the position.
- This position provides direct supervisory responsibilities for Pharmacy Technicians in the absence of the Pharmacy Director.
- May serve as coach and mentor for other positions in the department.
- Graduate from an accredited College of Pharmacy and licensed or eligible for licensure in the State of Minnesota.
- Three years’ work experience in an acute care facility is preferred, but not required.
- Post graduate residency in an acute care hospital is also preferred.
