If you are a RN who wants to be part of an amazing team, we want to meet you! We are currently hiring Per Diem RN Admissions Nurse to join our Home Care Team.

Responsibilities:

• 
  Develops a written plan of care that guides nursing intervention and facilitates desired outcomes.
• Implements interventions based on the plan of care in accordance with client and caregiver level of skill and knowledge.
• Evaluates client/ family responses to interventions and changes the plan as required.
• Document care provided and client/ family responses consistent to agency policy and procedure.
• Utilizes the Nursing Process, plans, directs and supervises the care of patients in addition to being involved in the provision of direct care of patients and families.
• Communicate effectively with clinical team members.
• Shall demonstrate knowledge of St. Camillus programs and services.
• Shall educate patients, families, on programs and services available through St. Camillus.
• Shall maintain an understanding of Medicare, Medicaid and all third party insurance payment terms.
• Follows agency policies and procedures.
• Communicates with physician and other disciplines regarding client status. Coordinates services as needed.
• Utilizes community resources in the development of plan of care.
• Shall maintain a current NYS Driver’s License and have a reliable vehicle to allow transportation between locations where services are rendered.
• Attends educational programs to maintain current standards of practice.
• Maintains agencies expected productivity standards.
