Aurora Mental Health & Recovery (AMHR)is hiring a full-time Peer Specialist to work with people experiencing homelessness and provide services through street outreach (this requires being out in the communities).
About the team.
Our Pathways to Home team (PATH) provides intensive community-based services to individuals and families experiencing homelessness. There are 4 primary components to the PATH team: Street Outreach - engage folks disenfranchised from traditional models of care; Drop In - folks come as they are to get access to a full continuum of services in the mental health and housing sectors; Community Based care - services to decrease barriers and increase engagement to long term teams; Housing team - housing case management continuum from assessment to successful completion or maintenance in housing stability.
About the role.
The Peer Specialist provides assessments for care coordination, resources, and linkage to Substance Use Disorder services through AuMHC. Additionally, the Peer Specialist provides milieu management and case management during Drop in. The Peer Specialist will perform a wide range of tasks to assist peers in regaining independence within the community and mastery over their own recovery process.
Responsibilities:
- Assist clients in articulating personal goals for recovery through one-on-one meetings and group sessions.
- Support clients in identifying and creating goals and developing recovery plans.
- Provide follow-up services with clients after their admission or assessment with Crisis Services for a successful transition to community-based services.
- Assist clients in working with their case manager or treatment team in determining the steps they need to achieve goals and self-directed recovery.
- Meet with clients in a wide range of settings (individually, group or family homes, community-based outpatient settings, community agencies).
- Assist clients in obtaining services that suit the recovery needs by providing community resources.
- Assist clinical staff in maintaining accurate data.
- Provide transportation for clients in personal vehicle and/or create access to services in another manner.
- Assist in navigation and advocacy with clients' appointments and attendance for criminal justice proceedings, medical, and other needs.
- Staff member will monitor and coordinate care for assigned clients in partnership with the other members of the clients care team.
Requirements:
- Valid Colorado Driver's License with a driving record that meets organization's requirements.
- High school diploma/GED.
- Lived experience with behavioral health concerns.
- Completion of a Clinical peer specialist or peer coach course.
- Preferred: Clinical Peer Specialist certification.
Required Vaccinations and TB Test.
AMHR has prioritized the health and safety of our clients and staff and requires all employees to be fully vaccinated against Covid-19, receive an annual influenza (flu) shot, and receive negative TB test results prior to their first day of hire. Proof of both vaccinations and negative TB test will be required upon your first day of employment. Medical and religious exemptions may be requested for either COVID 19 vaccine, annual flu shot or flu shot.
Salary for this role. Wage is based on experience and company equity.
$ 16.05 to $21.29 hourly
Additional Compensation Opportunities:
Language Differential Pay (For Bilingual Candidates): 5% of base salary up to $3,000/year
