Patient Navigator (RESEP)(Flagstaff, AZ)

North Country HealthCare is a non-profit health organization comprising of 15 clinics in 12 communities across Northern Arizona. We are focused on creating healthier communities and in ensuring that not one person is without access to Primary Care.

Position Description Overview:

The RESEP patient navigator is a part-time position. The patient navigator will facilitate RESEP exams, provide patient navigation for program-eligible clients, and provide claims assistance for the Radiation Exposure Compensation Act (RECA) claim. They are responsible for determining client eligibility, securing annual screening exams, providing exam results, and navigation for abnormal results. The patient navigator component is monitoring and documenting screening results and case-managing abnormal results appropriately. Additionally, the patient navigator will assist eligible clients with a federal compensation claim, the RECA Downwinder claim. In addition, the outreach component for this position includes presenting and promoting RESEP information to the public, and eligible communities and recruiting individuals to be screened in the program. The RESEP program's service area covers six counties in northern Arizona. The position requires strong organizational and communication skills and high attention to detail.

Overall Position Responsibilities

• Contact clients for annual exams by letters and phone
• Intake and verify program eligibility of new clients
• Oversee proper enrollment of participants and properly track information into database
• Maintain an accurate phone and client encounter log
• Ensure that medical staff have all paperwork and information needed for appointments scheduled
• Participate in planning, implementation, and evaluation of program's goals and objectives
• Responsible for patient navigation through RESEP exams and referrals
• Ensure that 100% of RESEP patients will be contacted regarding exam results and follow-up (normal and abnormal results) on time
• Track clients' appointments and screening and diagnostic outcomes in appropriate database and follow-up with clients as needed
• Provide RECA claims assistance to eligible clients and their families by distributing the RECA claim forms and checklist of required documents
• Effectively communicate the RECA eligibility requirements and claim process
• Travel throughout the Northern Arizona service area (Coconino, Yavapai, Navajo, Gila, Apache, northern part of Mohave) as necessary
• Communicate well with team and medical providers to ensure high quality care for our clients
• Fulfill the Scope of Work as dictated by funding organization(s)
• Support and develop relationships with clients and partners in the community
• Ask questions when necessary and provide feedback about processes and areas for program improvement to RESEP program manager, teammates, providers, and community partners

Compensation:

$20.58-$23.70 DOE
