When you join PPGNY as Patient Navigator for Abortion Services, the Abortion patient navigator focuses on the abortion patient’s needs to access care on both an individual basis, and at a systems level. The patient navigator helps guide the patient through the abortion care system and works to overcome obstacles faced by the patient receiving the care and services they require, while working to improve the systems and processes that shape access at the clinical and ecosystem levels. The Navigator will also provide administrative support.

Additional responsibilities:

    • Organizing schedules and managing appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them based on the patient’s clinical care site preferences.
    • Collaborate with diverse organizations and personnel to ensure the needs of the patient are met.
    • Facilitating communication between the patient, support person(s) and/or organization(s), and healthcare providers to ensure patient satisfaction and quality of care.
    • Informing the patient on financial aspects of care and linking the patient to financial resources as needed.
    • Working with the patient to identify logistical needs to access care and linking the patient to appropriate resources as needed.
    • Working with the patient to identify other support services as needed, and providing appropriate linkages
    • Facilitating pre and post-abortion visits and ensuring continuity of care and linkage to community resources
      • Identifying and addressing existing gaps and opportunities within existing patient navigation processes, including screening processes, to ensure highest-need patients are being identified, routed, and connected to navigational support
    • Working with external stakeholders and organizations to build and strengthen relationships, systems, and processes that support access to abortion care
    • Working with external vendors to facilitate access to key services, including but not limited to, transportation and lodging
    • Working closely with affiliate and contact center staff, including but not limited to social workers, Justice Fund managers and counselors, contact center staff, health center managers, and others to create efficiencies and improve workflows for increased abortion access and improved patient experience
    • Developing, managing, and advancing program goals and objectives for new and/or improved processes and systems that increase access to equitable abortion care

You have:

    • BA or BS in health or human service related field
    • Strong understanding of abortion and related health care facilities, support structures and processes.
    • Work experience working in an ethnically, culturally, and racially diverse environment.
    • Strong ability to manage clinically and emotionally challenging situations with utmost professionalism, care and discretion.
    • Ability to provide precise, well-written documentation.
    • Demonstrated computer skills (Microsoft Outlook, Word and Electronic databases)
    • Ability to travel to across all PPGNY regions

What makes you stand out:

    • Experience in community based medical/health or social service settings, social work, counseling, nursing, public health or other health education
    • Knowledge of regional resources
    • Experience and/or training in trauma-informed care, abortion access, case management, and/or program management

Compensation:

    • $65,00- 73,000 yearly

All positions at PPGNY require:
Proof of immunization or immunity to certain communicable diseases (including influenza during the flu season) and testing for tuberculosis. These certifications are required by the NYC DOHMH Health Code, NYSDOH and OSHA.
