Housing and Services, Inc. (HSI): HSI is a NYS nonprofit developer and manager of supportive housing that has been delivering lasting solutions to homelessness for over 35 years. Since 1987, HSI has added over 2,000 units to NYC’s affordable housing stock that are home to special needs populations including: very low-income seniors, the mentally ill, those struggling with substance use, people living with HIV/AIDS, youth aging out of foster care, veterans, and the co-diagnosed. HSI manages 722 units ongoing at our four housing programs: the Cecil, Narragansett, Kenmore Hall, The Bedford, and our Scatter Site Program.
We are seeking a patient navigator for our Cecil location that will help guide tenants through all aspects of scheduled medical and psychiatric appointments. You will be responsible for accompanying assigned tenants to their appointments, collecting information about follow-up care, as well as addressing any queries or complaints. Your ultimate duty is to ensure patients receive the best care available to them.
To be a successful patient navigator, you should have knowledge of the local healthcare systems and an ability to communicate complex information effectively. The best candidate will also be an excellent listener with a highly empathetic mindset.

Responsibilities:
- Scheduling transportation to and from scheduled appointments.
- Helping patients navigate check in procedures, forms, and insurance information.
- Documenting and resolving any complaints lodged by the patient.
- Reviewing paperwork for the patient to ensure it is accurate and complete.
- Explaining test results, diagnoses, and available treatments to the patient as well as the assigned Case Manager.
- Communicating all the information patients need to make informed decisions about their healthcare.
- Other duties as assigned.
Qualification:
- Associates or Bachelor’s degree in a healthcare-related field preferred.
- Excellent listening and communication skills.
- Empathetic attitude, with an ability to offer emotional support.
- Passion for the proper care and well-being of patients.
Job Type: Full-time
Pay: $45,000.00 - $51,000.00 per year
