Primary Responsibilities



Comments

Interprets physicians' prescriptions and dispenses medications for high dollar/specialty medications (e.g. Oncology, Multiple Sclerosis, and Infectious Disease) within regulatory guidelines.
Provides medication therapy management by actively managing drug therapy; identifies, prevents and resolves medication-related problems. (e.g. Counsels patients and/or medical staff and handles complex problem resolution).
Completes patient records, initial patient assessments, medication profiles, initial fill patient notes and continuing patient progress notes.
Participates in the coordination of care to meet patient outcomes/goals including, initial referral, medication review and selection, assessment of medication delivery system, coordinated care conferences, patient care plans, compliance programs and discharge of patients from services.

1. Reviews patient demographics and prescriptions issued by physician, or other authorized prescriber to assure appropriate therapy for the patient, accuracy of dose and determine formulas, ingredients, and medications/administration supplies needed for optimal patient outcomes.
2. Completes patient counseling in compliance with clinical program protocols and adhere to and counsel patients in compliance with REMS programs; educate patients on drug interactions, side effects, dosage, administration and storage of pharmaceuticals and supplies.
3. Ensures access to prescribed medications by coordinating with other specialty pharmacy departments, medical staff and insurance companies. Obtains, completes and submits prior authorizations, appeals and any other necessary patient/physician information to pay or for review and approval.
4. Reviews and supervises the work completed by pharmacy technicians engaged in all areas of the prescription process including processing, mixing/compounding, labeling, packaging, and claim submission, of pharmaceuticals and supplies.
5. Contributes to departmental Quality Assurance/Quality Improvement activities. Expected participation no less than 5% of time – increased participation based on need/project.
6. Interacts with co-workers, visitors, and other staff consistent with the iSCORE values of Jefferson.





PharmD degree or Bachelor of Science Degree in Pharmacy with 10+ years of pharmacy experience

PA license

Prior specialty pharmacy experience or residency preferred
