Description
Purpose: The primary purpose is to serve as a key point of contact between patients and medical providers to address patient concerns or questions that patients may have after receiving care. Will also oversee medication refill requests and scheduling of patient referrals. Must work well with providers to deliver healthcare services in a timely, accurate and pleasant manner to clinic patients.

Duties:

• Answering all incoming nursing calls to address patient questions. Making outgoing nursing calls to notify patients on behalf of the providers.
• Check phone messages and patient communication module periodically throughout the day.
• Complete outgoing phone calls and written correspondence as directed.
• Return all patient calls before going home each night.
• Address all patient medication refill requests daily. Including reaching out to patients to schedule appointments when indicated by the provider.
• Notifying patient of test results and dating and initialing report when done before it is scanned.
• Contacting patient about lab and radiology results as requested by providers.

• Patient Referrals to specialists.
• Schedule appointment time.
• Obtain any insurance prior authorizations as required for MRI’s, CT’s, etc.
• Send any chart notes, labs, or films needed for appointment.
• Provide patients with appointment date and time, directions, and any other instructions as provided by specialist.
• Track all referral appointments in excel spreadsheet per RHC requirements. Including patient name, referral to, reason, appt time and date, date referred, and any miscellaneous notes.
• Make sure all reports are received after patients visit with specialist.
• Send Healthy Connection referral if needed.
• File all referral forms in patients’ chart.
• Get other providers on the phone for our providers to discuss patient care with.
• Keep updated list of all area physicians with current phone numbers, fax numbers, and addresses.
• Track all providers new to the area and enter their contact information into the spreadsheet and notify providers.
• Enter referrals into Referral Management in EMR.

• Maintaining and monitoring for expiration the drug sample closet.
• Act as patient advocate in health maintenance
• Maintenance clean work environment for patients, personnel, and visitors.
• Assist other office personnel as directed by supervisor.
• Other duties as assigned by the clinic manager or their designee.

Work Conditions

• Frequent pushing and pulling movements
• Subject to frequent interruptions
• Constant interaction with other personnel, physicians, patients and public
• Communicates with other office personnel and other department employees

Qualifications
Abilities, Experience and Education:

• CNA, CMA or MA
• High School Graduate or GED
• Computer skills
• Good communication skills required
• Must speak, read, and write English language, competent bilingual English/Spanish preferred
• Excellent people skills
• Ability to organize and multitask

Development and Participation:
Attend PCHD clinic staff meetings and in-service education programs

Job Type: Full-time

Pay: $15.00 - $27.26 per hour
