Position Summary:

Our Patient Access Navigator team is responsible for performing all site front office functions including incoming and outgoing phone activities, appointment scheduling, patient registration, computer data entry, collection of payments, maintaining medical and dental records and serves as an example of excellent customer service to patients, family members, and community partners.

Your contribution to the team includes:

  • Greet clinic patients and visitors as appropriate, which includes checking in all patients by entering patient data into the computer system.
  • Ensure accuracy of all data including income, address, and insurance verification.
  • Collect patient fees and provide patients with receipts for the office visits and medication refills.
  • Ensure patient needs and requests are handled efficiently.
  • Schedule patient initial and follow-up appointments ensuring efficient use of provider time and the appropriate care for patients according to HealthPoint procedures including accessing and updating patient information.
  • Maintain schedule accuracy for maximum patient flow by checking schedule a minimum of five clinic working days ahead, confirming provider date and times are correct, identifying and correcting inaccuracies or mistakes to schedule, and ensuring that unique events and provider administrative time are accurately reflected on the clinic schedule.
  • Follow established procedures for answering and screening incoming appointment-related telephone calls, scheduling interpreters, directing calls to appropriate staff, and taking messages.
  • Maintain effective communication with back-office staff and providers as needed to appropriately "work in" additional patient visits on providers' schedules.
  • Distribute appropriate forms and provide instructions to patients according to HealthPoint procedures.
  • Enroll patients into special state programs as appropriate.
  • Coordinate and facilitate whole-person care for all services at HeatlhPoint Health Center.
  • Educate patients on the integrated primary care model and explain services available to them i.e BH, pharmacy, dental and medical.
  • Greet all clinic patients and visitors as appropriate, which includes checking in all patients by entering patient data into the computer system. Ensure accuracy of all data including income, address, and insurance verification.
  • Access and update patient information in Electronic Practice Management (EPM) and Electronic Dental Record (EDR) computer system as required.
  • Register and check-in patients.
  • Discharge patients and scheduling follow-up or other types of appointments.
  • Demonstrate respectful, professional, and appropriate behavior that supports a team-oriented work environment.

Must have’s you’ll need to be successful:

  • High school diploma or equivalent. Zero (0) to six (6) months of relevant experience; or equivalent combination of education and experience.

If you know about the following, it’s a plus:

  • Customer Service &/or Medical Front Desk/Receptionist experience -preferred.
  • Bilingual in English/Spanish preferred OR Somali/English OR Punjabi/English -preferred.
  • Willingness to learn and adapt to a quickly changing environment.

Proof of immunizations and/or immunity to: COVID-19, MMR (measles, mumps, and rubella), Varicella, Annual Influenza, and TB are required for all employees. Additionally, if you work in a HealthPoint clinic, Tdap (within last 10 years) is required. Hepatitis B. is required for clinical employees with potential exposure to blood/blood products. All immunizations are a condition of employment. Upon hire, employees must provide proof of their immunizations and/or immunity prior to starting or no later than their fifth (5) business day of employment.
