Job Title: Palliative & Hospice Care Physician
Job Summary
A Palliative and Hospice Care physician provides management of the medical component of the Palliative and Hospice Patient’s care. Ensures provision of services that reflects Hospice Maui’s philosophy and standards. Adheres to all state, federal, and other regulatory guidelines as applicable. Works in conjunction with the Medical Director and patient’s attending physician. Provides direct patient care in the Palliative and Hospice setting as needed. Establishes relationships with the medical community in order to increase awareness and provide education about palliative and hospice care. Participates in the agency performance improvement program as requested.
Essential Job Functions/Responsibilities - Responsibilities of the Palliative & Hospice Care Physician include but are not limited to the following:
- Consults with the Medical Director and attending physicians if needed, regarding pain and symptoms management for the Palliative Care and Hospice patients
- Certifies patients’ medical eligibility for Palliative and Hospice services based on a review of
~The primary terminal condition;
~Related diagnosis(es), if any;
~Current subjective and objective medical findings;
~Current medication and treatment orders; and
- Trains staff regarding the medical aspects of caring for chronic, advanced and terminally ill patients
- Acts a medical liaison with other physicians in the community
- Working together with an Interdisciplinary Palliative Care team, consisting of a Physician, Social Worker, and Chaplain
- Provide palliative care services to outpatient palliative care patients
- Providing comprehensive pain and symptom management.
- Communicates with the team to ensure the Plan of Care is followed and changes are approved.
- Approves team decisions requiring physician oversight.
- Conducts daily rounds at the local 250-bed hospital
- May round at the Long Term Care facilities where Palliative Care and hospice patients reside
- Attends Interdisciplinary Team Meetings to discuss patient care as needed/directed by the Medical Director.
- May conduct Face-to-face attestation visits to ensure patients continue to meet eligibility requirements for Hospice 60-day certification periods.
Position Qualifications (Knowledge, Skills, and Abilities)
- Education: Completion of medical education and relevant residency training required. Fellowship in Palliative Medicine preferred
- Experience: Minimum of 1 years work experience in internal medicine, geriatrics, palliative medicine, or other relevant field of medicine required. Work experience in a palliative medicine consultation practice or certified hospice program and administrative experience preferred
- License/Certification: Physician licensure by the State of Hawaii as a Doctor of Medicine or osteopathy required; board certification in hospice and palliative medicine preferred
- Valid driver’s license with an automobile that is insured in accordance with state and/or agency requirements and is in good working order required
Medical education and residency and Hawaii state licenses are required. Equivalent experience which provides proficiency in the areas of responsibility listed above may be substituted for the experience requirements.

- Office work would require the ability to lift files, open file cabinets, stand and bend
- The position requires intermittent physical activity including standing, walking, bending, kneeling, stooping, and crouching as well as lifting and supporting patients.
Work Environment
- Work will involve an office setting with routine use of office equipment such as computers, phones, photocopiers, fax machines, and filing cabinets and clinical settings at patient homes, community facilities and in-patient facilities.
Travel
- Travel is intermittent and primarily local during the business day
Continuing Education Requirements:
- Participates in appropriate continuing education as required.
- Accepts personal responsibility for other educational activities to enhance job-related skills and abilities.
- Attends mandatory educational programs.
