Job Summary:
Responsible for the daily operations of the palliative care program including, referral management, coordination of admissions, discharges and change of levels of care, management of ancillary vendors, maintaining patient records, coordination and preparation of meetings, and clerical functions. The Patient Care Coordinator is responsible for maintaining a high level of customer service with the patients, family members, provider, vendors, and referral sources.

Essential Qualifications: Current NH licensure as a Medical Assistant or Licensed Practical Nurse with at least 2 years of medical experience and training. Associates in Nursing, Bachelor's degree preferred. Three years in palliative, hospice, and/or home care; or a combination of education and experience that demonstrates the person has acquired the necessary competencies required for this position. Must have effective social interaction, communication and organizational skills and the ability to perform a variety of tasks efficiently. Must have previous experience with an electronic medical records system and be proficient in Microsoft office software. Must also be able to demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities. Access to operable, insured motor vehicle and a valid driver's license with and acceptable record of safe driving practice required. Must obtain CHPN certification within two years of hire.:
