Position Summary:
Under the direction of the PACE Transportation Supervisor and in conjunction with the contracted transportation provider, the PACE Transportation Coordinator is responsible for coordination of transportation services and ensuring the overall quality assurance of PACE Transportation Services. Responsible for coordinating, scheduling, and arranging necessary transportation services for all PACE participants and potential enrollees. Responsible for assisting PACE participants in accessing medical care, specialty appointments and referrals, as well as transportation to the day center. In conjunction with the contracted transportation provider, Responsible for coordinating, monitoring, documenting, and reporting the delivery of transportation services with transportation contractor and San Diego PACE Staff.

Essential Functions of the Job:
  • In conjunction with the contracted transportation provider, plans and coordinates all transportation scheduling activities related to San Diego PACE participants.
  • In conjunction with the contracted transportation provider, monitors the activities of the drivers/aides daily.
  • Responsible for coordination of all outside service providers of transportation.
  • Assures all necessary program records are maintained according to policy.
  • Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in the participant care planning. Communicates any changes in participant condition to team members.
  • Monitor transportation services, coordinates with other PACE Team members (Clinical coordinators, case managers, social workers, therapy staff), participants, caregivers, and schedules with time in advance any scheduled visits to include at a minimum date, time, place and contact person.
  • Responsible for transportation set-up arrangements, coordination of services, maintaining required documentation and ensure that participants arrive on time to their scheduled appointments.
  • Attend all interdisciplinary meetings.
  • Utilize transportation software to provide timely information to participants and PACE staff.
  • In conjunction with the contracted transportation provider, coordinates safe and timely boarding and de-boarding of participants as they are transported from home to the center and from the center to home. This may include assisting with assignment of buses, coordinating schedules routes, and accommodation of participants.
  • Coordinates, monitors, and maintains accurate records and data of day-to-day transportation activities and provides accurate reports to Program Manager as necessary.
  • Keep PACE and day program staff informed of any changes in schedule or locations.
  • Address any immediate service problems that may arise. Communicate concerns of clinical services to the PACE Manager.
  • Transport participants in PACE vehicle when needed.
  • Collaborate with office staff in keeping adequate records and files.
  • Develop and implement transportation Policies and Procedures.
  • In conjunction with the contracted transportation provider, ensures transportation services are delivered with the highest quality standards of services.
  • Provides high level of customer service.
  • Answers telephones and transportation inquiries in a timely manner.
  • Reports any incident immediately following PACE center protocols.
  • Ensures transportation service provider staff meet the PACE training and certification requirements.
  • In conjunction with transportation provider, develops and implements quarterly transportation surveys.
  • Maintains and complies with established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
  • Performs other related duties as assigned or requested.
  • Properly documents in Truchart all appointments, communication, and relevant information regarding participants’ appointments, follow ups and transportation needs.
  • Coordinates with specific team to address any participant needs (i.e companionship) during visit.
Additional Duties and Responsibilities:
  • Maintains and complies with established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
  • Performs other related duties as assigned or requested.
Job Requirements: Education Required (Minimum level of education):
High school diploma/GED

Certifications/Licenses Required:
CA driver’s license with appropriate automobile insurance.

Experience Required (Minimum level of experience):
  • A minimum of 6 months’ experience in a healthcare-related field.
  • Experience working with the elderly population strongly preferred.
  • Experience with transportation coordination strongly preferred.
  • Demonstrated ability to handle multiple ongoing department requests, ability to work under minimal supervision and ability to meet deadlines.
Verbal and Written Skills Required to Perform the Job:
Must be fluent in verbal and written Spanish and English. Excellent organizational and communication skills; Good public relations.

Technical Knowledge and Skills Required to Perform the Job:
Transportation delivery system preferred.

Equipment Used:
Personal computer, telephone, fax, copier and other office equipment. Must be able to perform computer proficiency.

Working Conditions and Physical Requirements:
  • Physical activities include prolonged periods of sitting while working at computer. Frequent walking and standing.
  • Driving of personal and/or company vehicle.
  • May be required to work outside of normal working hours on occasion.
