Overview:
The Outpatient Clinical Supervisor provides leadership, consultation and collaboration with the goal of facilitating the provision of high quality care and service to the patient. He/she serves as a resource to the organization and the community for information related to patient care. The Outpatient Clinical Supervisor must have an intense commitment to high level customer service and the ability to foster that culture within the site(s) they manage.

The Outpatient Clinical Supervisor has a direct reporting relationship to the Director of Clinical Operations with a dotted line reporting responsibility to the Director of Practice Operations.

The position will work in partnership with the Primary Care Practice Supervisor. He/she will work closely with physicians, medical directors (MD), Clinical Lead providers, nursing and other staff who interact and provide care to patients in the clinic. The Position may be asked to participate in standing committees related to practice of care impacting patients/operations in the clinic.

The Outpatient Clinical Supervisor has authority to direct, develop, and implement processes related to clinical care in collaboration with the Medical Director or other Clinical Leaders.
Responsibilities:
Quality and patient-focused care:
  • Supports optimization of patient access in collaboration with Practice Supervisor(s), Medical Director and/or other clinical leader.
  • Promotes hospital wide initiatives by providing & monitoring that best practice is being utilized through service excellence and risk management.
  • Responsible for maintaining competency for their specific unit with the expectation to work as team leader/staff nurse when applicable.
  • Leads and participates in staff huddles.
  • Support Population Health initiatives utilizing informatics to plan and drive strategies.
  • Ensure Joint Commission, Department of Health, National Committee for Quality Assurance (NCQA), Medical Home and Accountable Care Organization (ACO), and other regulatory agency requirements are recognized and observed.
  • Integrates and coordinates varied initiative aimed at improving performance and quality of care across department or organizational lines
Works within interdisciplinary team(s):
  • Responsible for the provision of patient care.
  • Facilitates multidisciplinary teams in initiatives that support improvement in the delivery of care.
  • Partners with an interdisciplinary team of professionals and ancillary staff to drive transformation of Primary Care in support of network wide and population health goals.
  • Works collaboratively with other departments within the organization to identify and address areas where potential breakdowns in the continuum of care may be occurring.
Leadership and professional practice skills:
  • Recruits, interviews and selects clinical staff.
  • Completes regular performance evaluations for clinical staff members.
  • As necessary provides performance coaching and counseling within the organization’s policy.
  • Develops and implements orientation tool/process as well as in-services/training; tracks nursing competencies and ensures completion of all mandatory training modules.
  • Works collaboratively with the interdisciplinary team of professionals and ancillary staff to ensure efficient workflows in the Primary Care setting.
  • Demonstrates effective conflict resolution skills
Communication and relationship building:
  • Serves as liaison for/with nursing staff.
  • Works with Director of Practice Operations, Director of Clinical Operations and Medical Director to ensure provider/staff satisfaction and retention.
Qualifications:
  • Bachelor of Science degree in Nursing required.
  • Current New york State Registered Nurse licensure required.
  • Three (3) to five (5) years of progressive leadership experience required.
  • Three (3) years of clinical experience in a physician office setting preferred.
  • A minimum of two (2) years using an Eletronic Medical Record - GE Centricity or EPIC preferred.
  • Experience in Population Health preferred.
  • Pediatric experience preferred.
