Summary
  • This position is responsible for the enrollment, day-to-day management, and coordination of remote monitoring services, working in close collaboration with the care managers involved in the patient's care. This position will monitor patient health data collected via remote monitoring equipment, interfacing with patients via phone or videocall to conduct assessments when needed and collaborate with ordering providers to promote good patient outcomes based on established protocols. Additionally, the RM nurse will coordinate with providers to obtain orders for remote monitoring, interface with Health Recovery Services (HRS) to ensure that equipment is delivered to patients and liaise with local and regional providers to encourage use of the system to improve patient outcomes and reduce readmissions. This position works under the supervision of the director of outpatient care management and is employed by Mosaic Health Systems.
Duties
  • Responds to referral requests in a timely manner and offers possible alternatives for assistance if referral is not appropriate for RM program.
  • In conjunction with the patient/family, provider, care management, and interdisciplinary team, sets goals and prioritizes remote monitoring plan to achieve optimal outcomes (Quality, Cost, Experience).
  • Through the nursing process, using specific protocols and established resources collects specific data and records information in database/documentation form, assesses the client’s condition, provides accurate consistent information, and validates the information and plan with the client for completeness and accuracy.
  • Renders sound clinical recommendation based on ability to assess and prioritize clinical needs as presented through telephonic interview. Is competent in recognizing the gravity of a healthcare problem and able to act upon urgent medical problems, while providing safe, effective, and appropriate care. Works collaboratively with other team members as required in developing and implementing the plan of care.
  • Coordinates/facilitates implementation of plan demonstrating problem solving skills, effective communication and appropriate sense of urgency in providing service to avoid over, under, or inappropriate utilization of resources (including but not limited to unnecessary ED/IP visits, unplanned readmissions, unnecessary testing/treatment, etc.).
  • Other duties as assigned
Qualifications
  • All required education is a minimum requirement. Higher levels of education are acceptable. Bachelor's Degree, Graduate from an accredited School of Nursing required.
  • RN - Registered Nurse - State Licensure And/Or Compact State Licensure and Driver's Licence - Valid And In State required.
  • 3 years emergency department experience, ambulatory nursing, critical care or medical surgical nursing and experience navigating resources and computer systems to access information required. Previous telephone consultation experience and multilingual skills preferred.
