Job Description

We are seeking a highly-skilled, licensed occupational therapist (OT) to join our growing healthcare team.

We are looking for compassionate, knowledgeable, and caring Occupational Therapists who want to impact our members’ quality of life.

In this role, you will assess and evaluate members’ needs and requirements to achieve and/or maintain their health. You will guide members and their families toward and facilitate interaction with resources appropriate for their care and well-being.

You will also work in collaboration with a multi-disciplinary team, employing a variety of strategies, approaches, and techniques to enable a member to manage their physical, environmental, and psycho-social health issues.

General Duties/Responsibilities:

(May include but are not limited to)

  • As a member of the Wellness at Home Team, will provide occupational therapy to seniors by conducting home and residence visits as assigned, including home safety and wellness assessments, care planning, and ongoing re-assessments of member care to support immediate and long-term care needs.
  • Involves the members’ families and primary caregiver(s) as needed to achieve the best care decisions and outcomes.
  • Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to their supervisor/team. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors, and others.
  • Consistently meets or exceeds WAH targets for productivity, customer service, quality assessment, and performance targets.
  • Understand the importance of community involvement and participate appropriately in activities linking WAH to its communities.
  • Maintains currently written progress notes and other documentation on the member's Medical Record including signed and dated documentation for all services performed the day the service is provided.
  • Complies with all policies and procedures of WAH.
  • And other tasks, as assigned.

Minimum Requirements:

Education:

  • Master’s Degree in Occupational Therapy; Master’s Degree from an accredited school preferred. Must have OT license.

Experience:

  • Two years of Occupational Therapy experience, preferably in home health care, and a minimum of one-year of experience working with a frail/elderly population.
  • Age-specific competency in working with the elderly and knowledge of community services for the elderly and their families.

Other:

  • Valid Oregon driver’s license with a good driving record.
  • TB screening and successful completion of a health examination by a hospital-designated nurse practitioner or physician are required for employment.
  • Requires up to 80% travel to member homes and residences, which may include nursing homes, assisted living facilities, and board and care facilities.
  • Excellent verbal and written communication skills
  • Ability to work effectively in a team environment.

Job Types: Full-time, Part-time

Pay: $45.00 - $55.00 per hour
