POSITION SUMMARY
Job Summary: This position provides therapy services to patients of all ages, including evaluation and assessment, individualized interventions, education, treatment planning, and consultation to improve patients’ abilities to their highest possible level. This position provides services as part of a care team committed to optimal clinical outcomes, under the direction of the referring physician. As a casual OT, this position is needed for weekend and weekday coverage.
POSITION QUALIFICATIONS
Education and Experience
BA/BS, Master’s, or Clinical Doctorate from Accredited Occupational Therapy Program
Occupational Therapy license to practice in the State of Minnesota
Certification from the National Board of Occupational Therapy
BLS/CPR certification
License/Certificates
Occupational Therapy license to practice in the State of Minnesota
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Performs initial evaluations/assessments and re-assessments per medical provider orders and within facility policy. Responsible for gathering information and data interpretation, which demonstrates knowledge and skills necessary to provide appropriate care.
- Develops and revises plan of care as indicated by patient diagnosis,
physical and cognitive status, functional goals/outcomes, and discharge plan.
- Provides treatments to meet individual’s needs (physical, cultural and
psychosocial) per plan of care.
- Patient/Family education: Plans for and provides patient, family
and caregiver education, identifies learning needs, clearly communicates information, and documents response to education provided.
- Completes daily documentation and billing according to current standards and policies.
- Adheres to hospital Joint Commission, Department of Health, and Medicare Standards.
- Discharge Planning: Plans for discharge, anticipates long term and short term patient needs, collaborate with interdisciplinary team, and assesses patient prior to discharge as needed.
