Our Stepping Stone to Recovery location has an exciting opportunity and new position for a Nursing Transition Planner. The significance of this role is to ensure our clients are getting the continuum of care they deserve and help bridge the gap in communication with nursing and clinical staff, as well as outpatient programs and agencies who will see the patient after they discharge from a residential level of care. The individual chosen for this position will blend the knowledge of CNA skills with medical case management skills.


Duties/Responsibilities:

  • Take vitals during med pass times
  • Phlebotomy
  • Create lab requisitions
  • Ensure clients Hx and Labs in EMR system are up to date
  • Transferring clients in EMR system
  • Discharging clients in EMR system
  • Placing clients on the transportation schedule for appointments while in treatment
  • Placing clients on medical providers schedules
  • Ensure UDSs are packaged and mailed
  • Meet with clients throughout their stay in residential to discuss medical needs.
  • Set up MAT appointments for clients discharge (Suboxone/Sublocade/ Vivitrol)
  • Set up medical appointments for clients discharge
  • Fill out Prior Authorization forms and fax them when needed
  • Obtain information from treatment team meetings and utilize it for that clients discharge
  • Help nursing keep track of inventory and obtain supplies
  • Other duties as assigned

Skills/ Abilities:

  • Knowledge of EMR systems and how to document data
  • Basic medical terminology
  • Phlebotomy
  • Vitals
  • Great customer service skills both, in person and on the phone to schedule appointments and set up aftercare
  • Empathetic and open communication skills
  • A passion for those struggling with addiction
