POSITION SUMMARY:

The Nursing Assistant, under the direction and supervision of the Registered Nurse, performs a variety of direct and indirect activities in caring for the personal and comfort needs of patients according to Standards, Policies and Procedures of the Carney Hospital. He/she reports to the Clinical Manager.

KEY RESPONSIBILITIES:

Responsibilities & Standards:

  • Age Appropriate Care:Utilizes knowledge of the principles of growth and development over the lifespan to provide direct patient care as assigned under the direction and supervision of the RN and according to shift routine and Nursing Policy and Procedures.
    Provides care as appropriate to patients with age specific related needs from adolescence through adulthood and geriatrics
  • Activities of Daily Living (ADLS)Bathing (including shaving and oral hygiene)
    Elimination
    Hydration/ Nutrition
    Mobility including ambulation, assists with patient repositioning to provide maximum comfort, aids Transport and transports patients as needed
    Assists patients with meal and feeds patient as necessary
    Notifies RN of any issues related to ADLs such as skin issues, incontinence, mobility and swallowing.
    Provides post-mortem care
  • SafetyParticipates in handoff from licensed staff and provides handoff to all members of the healthcare team.
    Assists RN with implementing Fall Prevention Strategies such as bed/chair exit alarms, use of fall bracelets and nonskid slippers
    Provides safe care to patients at risk for falls, does not leave patients at risk unattended on commode or toilet
    Rounds on patients hourly with attention to toileting, pain, positioning and location of personal items.
    Consistently keeps patient's bed in low position with side bed rails up, bed locked and call light within reach.
    Responds to call lights promptly
    Performs observation for one to four patients for safety.
    Performs observation for one to one observation for patients at high risk for suicide or patients in restraint for violent or self-destructive behavior.
    Maintains a direct line of sight with the patient, visualizing patients using the bathroom, sleeping and during visits and during meetings with members of the healthcare team.
    Notes changes in patient status and reports these immediately to the RN. Examples include the patient is snoring or sounds/appears to have difficulty breathing, becomes very quiet or motionless.
    Recognizes patient emergency situations and calls for help. Utilizes Code Blue and Rapid Response button as necessary.
    Documents patient safety observations every 15 minutes on Patient Observation Monitoring Tool.
    Aids in the application and release of restraint under the direction licensed staff.
  • Documentation
    Consistently and accurately obtains and records the following information for patients as assigned according to shift routine and policy. Reports abnormal/unusual results to RN immediately.Intake & Output
    Vital Signs
    Weights/other parameters as directed
    Collects specimens for laboratory testing and instructs patients as necessary. Collection shall include labeling and placing (with requisition) in hazardous bag in appropriate place.

  • Demonstrates familiarity with intranet for information regarding Employee Health, Human Resources, MSDS and other job-related links

  • Infection Control.Demonstrates knowledge of and adheres to all infection control procedures including handwashing techniques, universal precautions, dwell time of cleaning products and pre-cleaning of reusable equipment.
    Assist in keeping the unit and patient care areas, clean and orderly on a daily basis and ongoing as needed.
    Stock carts and empty laundry hampers prn during shift and at end of each shift as assigned.
    Cleans equipment and tags items according to policy (e.g. commode, EKG, I.V. poles)
    Prepares patient rooms for admission including orientation to the room.
    Upon discharge, removes linens and any supplies prior to housekeeping clean.
  • Communication/interpersonal skills.
  • Answers telephone in courteous manner, identifies self, relays messages accurately.
  • Greets and assists staff, visitors, patients, as needed throughout the hospital.
  • Reports on/off to RN at change of shift and whenever leaving nursing unit. Report should include all pertinent information via SBAR format.
  • Exhibits self-directed behavior and works co-operatively with the RN and other co-workers to complete assignments. Organizes work to complete tasks in a timely manner.
  • Demonstrates a willingness to co-operate, receptiveness to new ideas and accepts constructive criticism and/or assistance readily.
  • Exhibits behavior that is not abrasive, rude, abrupt, domineering or condescending when interacting with patients, visitors or other employees.
  • Exhibits enthusiasm and initiative in performing assigned tasks and demonstrates productive use of free time by taking on additional tasks without being asked.
  • Projects poise and a positive image. ID Badge must be worn while at work.
  • Protects organization’s values by keeping patient information confidential.

REQUIRED KNOWLEDGE & SKILLS:

  • Knowledge and skills necessary to provide care appropriate to patients ranging in age from adolescence through adulthood and geriatrics.
  • Increases job knowledge by participating in educational opportunities and completed all associated Steward University modules.

EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:

  • Education: High school graduate or equivalent. Documented completion of training program with six (6) months experience or enrolled in Nursing Program with at least one (l) semester completed
  • Experience: 6 months related course work/experience required
  • Certification/Licensure: CNA preferred. Current BLS certification through either AHA or Red Cross required.
  • Software/Hardware: Meditech preferred.
