Nurse Practitioner Assessor for Disabled Client Assistance Program at Department of Social Services Nassau University Medical Center Part Time

Nassau University Medical Center has a great part-time opportunity for Looking for a Nurse Practitioner to evaluate medical documents for all potentially disabled clients.

In this role, As a Department of Social Services Nurse Practitioner you will:

· Review Medical Forms completed by outside Physicians to determine employability, housing restrictions, daycare eligibility of clients.

· Identify the severity of the client’s illness

· Determine medical care required and steer clients to appropriate specialists

· Determine if a medical or mental health condition meets the SSA listing for DCAP nurses to file Supplemental Security /SSD applications for potentially disabled clients.

· Flag clients for substance use disorder and refer to a Behavioral Health Counselor

· Review the Home Care evaluation done by nurses in the State UAS program and Physical paperwork. *Implement Medicaid restriction program as directed by NY STATE

· Prepare and attend fair hearings as needed.

We require:

· Current NP certification and licensing.

· Strong computer skills
