This Nurse Navigator will work with neuro oncology patients to provide care from diagnosis through course of treatment and follow-up. This Nurse Navigator will see patients in clinic and inpatient settings as scheduled while also managing a patient caseload from an outpatient standpoint to assist in coordinating care, triage calls and manage other aspects of neuro-oncology care. The ideal candidate is CPHON certified, is highly motivated, works well with team members and is organized.
The Nurse Navigator also assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each cancer patient including support for interdisciplinary and cross facility collaboration including, but not limited to tumor boards, conferences, multidisciplinary clinics, and referral communication.
On the aggregate level, the Nurse Navigator will track performance of the program in line with NCH, division and facility goals, Children's Oncology Group, APHON, and NCCN practice standards, and identify opportunities to streamline care practice through development of new patient care tools utilizing evidenced based guidelines and resources.

The Oncology Nurse Navigator implements and supports the philosophy, mission, values, standards, policies and procedures of Nationwide Children?s Hospital and the Patient Care Services Division.
1. The Oncology Nurse Navigator performs care coordination core functions which include screening and identification of patients, establishment of an effective relationship with patient/family, assessment of patient/family strengths and needs, development of a family-centered plan of care, procurement and coordination of services, and monitoring and evaluation of progress
2. Facilitates the patient/family/guardian?s ability to assume responsibility for managing their own health care when appropriate by promoting return of the patient/family to the highest level of wellness possible, identifying appropriate services by appropriate resources at the appropriate time, assisting in the acquisition or verification of financial /payer/insurance information when appropriate, managing communication from the patient/family in the area of their expertise in between visits, hospitalizations, etc.
3. Assesses child/family needs for care coordination oversight/intervention. Identifies appropriate resource to facilitate continuity of care and optimal patient outcomes.
4. Facilitates and coordinates quality patient care in a timely and cost-effective manner while promoting multidisciplinary collaboration between all members of the health care team.
5. Acts as a resource to members of the health care team in providing patient, family, and staff with information regarding diagnosis, treatments, side effects, and current therapeutic trends in specialty area.
6. Coordinates and mobilizes health care and community resources to meet the needs of patients and their families through referral, consultation, education and/or care conferences.
7. Participates in clinical guidelines and outcome indicators for a given population. Analyzes patient and aggregate variances and recommends changes to improve outcomes. Aware of evidence based standards and applicable national standards.
8. Collaborates with members of the health care team in identifying learning needs, planning for, and providing instruction in the care of the patients and their families across the continuum. Coordinates health promotion, prevention services and education.
9. Provides consultation for clinical practice and program development within areasfof expertise.
10. Serves as a resource and liaison between Nationwide Children?s Hospital and the community regarding health care information in the area of specialization.
11. Participates in product identification, evaluation, and clinical testing in area of clinical specialization.
12. Participates in the evaluation of peers for recognition and development
13. Provides and recommends input for policy and procedure development for the Hospital and Patient Care Services Division in area of expertise.
14. Analyzes clinical trends and data to identify patterns for continuous quality improvement studies, research development and/or evidenced based practice.
15. Collaborates with program leaders to establish and/or maintain program goals and program budget as appropriate. Provides care in a fiscally responsible manner.
16. Maintains accurate documentation in the medical record of assessments, patient plans, interventions and patient response to intervention, and education provided to patients/families.
17. Serves as a patient/family advocate in meeting patient or family needs.
18. Actively pursues education and knowledge related to the Oncology nursing profession and their area of expertise.
19. Maintains awareness of research affecting patient populations in their specialty area. Evaluates and implements evidenced based practice as appropriate. Work closely with research team in care coordination.
20. Identifies need for plans and/or provides continuing education programs in specialty area and develops/coordinates appropriate educational resources.
21. Participates in orientation and continuing education of practitioners, staff and students as appropriate.
22. Participates in ongoing education related to information technology requirements for the department. Demonstrates competency in EPIC and mainframe computer applications and software needed to perform job functions.
23. Facilitates and coordinates ongoing educational needs of patients/families.
24. Mentors clinical staff to foster their personal and professional growth.
25. Demonstrates the ability to triage and manage treatment side effects over the phone.

Qualifications:
KNOWLEDGE , SKILLS AND ABILITIES REQUIRED
1. Current licensure as a registered nurse in the State of Ohio required.
2. Bachelor's Degree in Nursing required.
3. Certified Pediatric Hematology Oncology Nurse (CPHON)
4. Clinical skill and knowledge in the nursing care of children including normal growth and development and/or clinical expertise in specialized area required.
5. Three years related clinical experience in area of specialty desirable.
6. Effective customer service, communication, and interpersonal skills required.
7. Demonstrated leadership and teaching abilities required.
8. Professionally appropriate analytical and organizational skills required.
