The creation of the HHC System Support Office recognizes the work of a large and growing group of employees whose responsibilities are continually evolving so that we and our departments now work on behalf of the system as a whole, rather than a single member organization.

JOB SUMMARY

The Nurse Navigator serves as an advocate for, and liaison between, patients and providers. The Navigator exists to improve the patient experience and is responsible for providing education, counseling and care coordination to Tallwood Institute patients.

JOB RESPONSIBILITIES

• Assists with provider-to-provider hand-offs and facilitates scheduling of patient appointments
• Liaises with providers (and PCPs as needed) to facilitate referrals to specialists and services
• Receives and manages phone and online inquiries and directs patients and customers to appropriate services
• Contacts, schedules and follows up with patients' referrals to specialty appointments
• Identifies care access points for patients who are not clear about services/disciplines needed
• Directs questions about care planning to nurses/clinical staff supporting the respective specialties/providers
• Manages data and surveys (e.g., Men's Health Checklist) to identify and solve for gaps in the continuum of care
• Participates in relevant meetings (e.g., Clinical Councils, Patient Advisory, System Navigator team meetings)
• Participates in professional and general public community outreach events; facilitates community lectures
• Utilizes EMR; mines, manages and reports out on data (e.g., provider referral patterns, website inquiries)
• Maintains a comprehensive understanding of treatment options and assists in the development of clinical protocols
• Serves as a clinical resource and assists in program curriculum and staff development
• Serves as program advocate and liaison for patients, their families and their providers; promotes Tallwood and HHC services
• Performs other duties as assigned

Education:

• BSN

Experience:

• 5 years nursing experience

Licensure, Certification, Registration:

• Licensed as a professional RN in the state of Connecticut
