The Nursing Manager of Transitional Care Services will be working for a hospital that is part of a growing and financially stable non-profit medical center. The medical center has been serving highly diverse 1.5 million residents in surrounding communities with a comprehensive range of inpatient and outpatient services for over a century.

Reporting to the Director of Nursing, the Nurse Manager of Transitional Care Unit will be responsible for ensuring effective delivery of evidenced-based practice of clinical patient care and compliance with administrative and clinical policies and procedures in the Transitional Care Unit. The Skilled Nursing Facility Nurse Manager will oversee 68 FTEs.

The Post Acute Care Unit Nurse Manager will utilize Tranformational Leadership qualities including visionary capabilities and abilities to empower others with this vision, focused on meeting the needs and values of followers to motivate high performance in order to assure organizational success and goal attainment.

The Transitional Care Unit Nursing Manager is a distinct part of the hospital's leadership. This very active TCU has 45 beds with an average daily census of 40. They turn over 120 patients per month. There will be a new expansion underway soon and additional staff will be hired. The Transitional Care Unit Nurse Manager will assist with the future direction of the unit.
