The Position:

The Nurse Coordinator is responsible for managing the provision of individual patient services and coordinate/collaborate with the clinical staff to develop the patient rehabilitation plan. Provide follow-up skilled nursing visits for patients in compliance with the state Registered Nurse Practice Act. Monitor the provision of rehabilitation service/outcomes and communicate regularly with the patient, family, physician and payers. Work under general supervision. Responsibilities include:

• Coordination of patient care with internal and external team members
• Fax tracking to include monitoring of pharmacy/nursing fax que
• Document management to include assignment of patient related documents to the medical record
• Initiate new referral activities and verify patient information to include demographics, special considerations, medication verification and documentation, vascular access verification
• Coordinate placement of central vascular access as indicated
• Coordinate nursing care with external agencies as needed
• Coordinate and maintain start of care folders
• Post infusion follow up calls to patient to determine effectiveness and patient satisfaction.
• Manage all agency nursing documentation and initiate nurse billing
• Manage nursing orders by ensuring orders are signed and returned in a timely manner
• Office duties to include answering phones, managing incoming and outgoing mail in conjunction with office assistant
• Approval of timecards and expense reports for nursing staff
• Assist nurse manager with annual training and competencies
• Recruitment of nursing staff
• Special projects as assigned

Schedule:

• Full-Time, Typical Business Hours

• Home care or hospital discharge planning experience
• Home infusion or home health experience required
• Scheduling experience
• Experience with Microsoft Office (Word, outlook, excel)
